DMRs, COCs, ORC visitation logs, calibration logs and bench sheets were reviewed for the period April 2021 through July 2021. At permit renewal on March 1, 2020, the percentage for toxicity sampling changed and it was noted during the inspection the facility failed to make note of this change and used the older percentage in the past six toxicity samples.
