RE: September 2021 Monthly Tier III Discharge Monitoring Report
Attached are the analytical results and associated chain of custody for the September 23 2021, monthly
Tier III stormwater outfall sampling event, submitted in accordance with Section D of the NPDES
Stormwater General Permit No. NCG030000 and Certificate of Coverage No. NCG030027 issued to our
facility. Additionally, attached is the applicable Discharge Monitoring Report (DMR) Form provided by the
North Carolina Division of Water Quality (NCDWQ).
I certify under penalty of law that this document and all attachments were prepared under my direction or
supervision in accordance with the system designed to assure that qualified personnel properly gather and
evaluate the information submitted. Based on my inquiry of the person or persons who manage the system,
or other persons directly responsible for gathering the information, the information submitted is, to the best
of my knowledge and belief, true, accurate, and complete. I am aware that there are significant penalties
for submitting false information, including the possibility of fine and imprisonment for knowing violations.
