Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application: Existing stream UT -Boone Creek piped years ago. Evidence of pipe failure appeared during 2013 summer rains. Three separate but adjoining properties under two different owners make up the project area. These properties are located in an urban setting with mixed residential and commercial property uses. Two of the properties have commercial businesses located on their boundary; the third property is a vacant lot. Temporary ground cover, silt fence, asphalt waterbar, and a retaining wall have been installed since the pipe failure in August of 2013 in order to prevent worsening conditions and loss of property. 3b. List the total estimated acreage of all existing wetlands on the property: 0 3c. List the total estimated linear feet of all existing streams (intermittent and perennial) on the property: 0 3d. Explain the purpose of the proposed project: The purpose of the project is to replace an underground 24" CMP, which failed in August of 2013, and stabilize conditions on the three project properties. 3e. Describe the overall project in detail, including the type of equipment to be used: The Molly Hundley Pipe Repair project proposes to replace a failed underground 24" CMP which carrys jurisdictional waters, UT -Boone Creek. The pipe failure initially caused the ground to collapse in several places and a slope to fail between two of the properties. Temporary measures have been taken to stabilize conditions as described above, in section 3a. Immediate action is required to avoid further problems. The proposed pipe replacement will provide a permanent solution to the problem by installing new 24" HDPE pipe parallel to the existing failed 24" CMP. Flow will be maintained in existing pipe during the installation of parallel pipe. Flow will be pumped around work area during tie-in phase. Upon completion of pipe installation all distrubed areas will be permanently stabilized. Total length of pipe to be replaced: 313 If; total length of pipe to be installed: 310.97 If (an additional five 6' square junction boxes used to tie four pipe segments together as part of proposed plan). Typical site grading equipment will be used during the project including backhoe and dumptruck. Site work will be completed before the start of the trout moratorium.
