Turn left onto Deep Branch Road (immediately after passing the Department of Correction facility, travel straight approximately five miles. The Pembroke Wastewater Treatment Plant will be on the left. 6. Describe receiving stream features and the pertinent downstream uses: The discharge from the Pembroke wastewater treatment facility flows to Lumber River, at a point with a 7Q10 of 120 cfs flow (as according to the Whole Effluent Toxicity Testing Self -Monitoring Summary --- see attachment). It is classified as Class WS-IV, B, Sw, HQW waters, which promotes aquatic life propagation and survival, fishing, wildlife, primary recreation, and agriculture, and waters used as. sources for potable water supply. PART II - DISCRIPTION OF WASTES AND TREATMENT WORKS 1. a. Volume of wastewater to be permitted: (Ultimate Design Capacity) b. What is the current permitted capacity of the Wastewater Treatment facility? 1.33 MGD 1.33 MGD c. Actual treatment capacity of the current facility (current design capacity)? 1.33 MGD d. Please provide a description of the existing or substantially constructed wastewater treatment facility: The raw wastewater flows into the Influent Pump Station (with three each 36 HP pumps). Then, flows through a Mechanical Bar Screen (or a Manual bar screen), through Pre -aeration and Grit/Grease removal. Then, flows through a splitter box to two oxidation ditches, through two clarifiers. And; on to the chlorine contact chamber and dechlorination, and finally the Cascade Step Aeration. The facility has a 500,000 gallon digester and a 80,000 gallon sludge holding basin for sludge management. Sludge is aerobically digested to meet vector and pathogen reduction. Once the digester is full and decanting is complete, testing begins for Class B Sludge. Sludge is land applied under permit WQ0013729. This is normally a six-month process. The sludge holding basin is for extra storage. The facility has a secondary plan with Atlantic Dewatering for composting. e. Possible toxic impacts to the surface waters: None 2. Based upon the previous Compliance Evaluation Inspection (CEI) that was conducted on October 13, 2008, all units were in service, and "this facility continues to maintain a very high standard for wastewater treatment".. 4.: This Office recommends continuation of the Special Conditions for the Whole Effluent Toxicity monitoring. 5. Box 866 Pembroke, NC 28372 Subject: COMPLIANCE PIOM INITORINO INSPECTION Town of Pembroke Wastewater Treatment Plant NPDES Permit No. NC0027103 Robeson County Dear Mr. Cummings: Enclosed you Will find a copy of the Compliance Biomonitoring Inspection report for the inspection conducted during the week of October 13, 2008. The inspection included the same objectives as that of a routine Compliance Evaluation Inspection plus an Aquatic Toxicity (AT) test to evaluate the biological effect of the facility's discharge on test organisms. As part of the inspection a tour of the Wastewater Treatment Plant was conducted. All observations and recommendations are in Part D: Summary of Findings/Comments of this inspection report. The DWQ-Fayetteville Regional Office collected Outfall 001 effluent 24-hour composite samples on October 14 and October 17 use in a chronic Cerio daphnia dubia pass/fail toxicity test. The sampleswere sent to the Division of Water Quality Aquatie Toxicology Laboratory, located on Reedy Creek Road in Raleigh. The ' aquatic toxicity analysis resulted in an "Invalid" due to >20% control organism mortality. Additionally, on October 14, 2008 the above mentioned 24-hour composite was analyzed by the Division of Water Quality Chemistry Laboratory (located on Reedy Creek Road in Raleigh) for the following parameters (that are on the NPDES peinuit): Biochemical Oxygen Demand (BOD) = 2 mg/L, Total Suspended. Solids (TSS) = 6.2 mg/L, Ammonia as nitrogen = .15 mg/L, Total Kjeldahl Nitrogen = 0.50 mg/L, Nitrite plus Nitrate as nitrogen = 9.8 mg/L, and Total Phosphorus = 1.7 mg/L. This facility appeared to be well operated and well maintained. The June 2008 DMR had a very minor data transpose error: the correct TSS for June 17 was 3.0, not 2.0 as recorded on th'e DMR. As according to Part II, 'Section E, (8): "Where the pennittee...submitted incorrect infoimation...in any report to the Director, it shall promptly submit such facts or infoiniation." An amended Discharger Monitoring Report should immediately be sent to the Division. The following procedure should be followed in submitting the corrected reports: A. All corrected data should bewritten in red ink or hi -lighted. B. At the top of the Effluent Page, the words "Corrected Report" or "Amended Report" should be written in red ink on all copies. This will differentiate between the originally submitted report and the corrected report. C. Two (2) copies of the corrected report should be submitted to this Division and one (1) copy must be retained by the permittee.
