Enclosed is a report for the inspection performed on September 30; 2004 by Mr. Norm Good.
Where deficiencies are cited in this report, a response is required as well as for all lettered comments
and/or recommendations. Within thirty days of receipt, please supply this office with a written item for
item descriptionof how these deficiencies, comments and/or recommendations were corrected. If the
deficiencies cited in the enclosed report are not corrected, enforcement actions will be recommended.
For certification maintenance; your laboratory must continue to carry out the requirements set forth in
15A NCAC 2H .0800.
We apologize for the delay in issuing this .report. Mr. Good neglected to write the report for.
eight months. This is not our policy and we striveto get reports out within 30 days of the inspection.

I. INTRODUCTION:
This laboratory was inspected to verify its compliance with the requirements of 15A NCAC 2H .0800
for the analysis of environmental samples.
II GENERAL COMMENTS:
The laboratory is spacious and well equipped. All facilities and equipment are well maintained.
Overall documentationis well kept and most .data looked good. However, some further quality
control procedures need to be implemented. The guidance document for coliform counting has
been edited slightly since the lab was last visited. A new version of this form is included with this
report
III: DEFICIENCIES, REQUIREMENTS. COMMENTS AND RECOMMENDATIONS :
BOD:
1. Deficiency: Lab occasionally had reported .samples below 2mg/L.
Requirement: A lower detection limit of 2mg/L is established by the requirement for a minimum DO
depletion of 2 mg/L. Ref: Standard Methods 18th ed. 5210 B (6) (b).
IV. PAPER TRAIL INVESTIGATION:
Paper trail investigation consisted of comparison of laboratory bench, sheets and chain of custody
documentation to recent discharge monitoring reports. No errors in transcription or inconsistencies
were observed.
CONCLUSIONS:
Correcting the above cited deficiencies and comments and implementing the recommendations will
help this lab to produce quality data and meet certification . requirements. Please respond to all
numbered deficiencies and lettered comments.

Your letter, received May 12, 2005 concerning corrective actions for your analytical procedures has been reviewed.
The quality control measures taken in reference to the deficiency cited in the September 30, 2004 inspection report are
acceptable. However, this report does not attempt to comprehensively address all certification requirements prescribed in the
North Carolina Administrative Code (15A NCAC 2H .0800), promulgated methods, and applicable Code of Federal
Regulations (40 CFR Part 136). It is the responsibility of the certified laboratory to address any required changes in
Wastewater/Groundwater Laboratory Certification application information, and to correct the Laboratory Standard Operating
Procedures and the Laboratory Quality Assurance Manual when requirements of the above cited codes are not in compliance.
You reported that the updated attachment showing the addition of C-BOD was not received in the previous correspondence.
Another copy is enclosed.
If a future inspection should reveal that the deficiency cited in the inspection report was not corrected, enforcement actions will
be recommended in accordance with 15A NCAC 2H .0800.
