Enclosed is a copy of the site inspection report. The system is online and performing at levels consistent with the active permit for this facility. Laboratory data indicate the system is being effective in the reduction of the Chlorinated Solvent constituents. The continued monitoring of the system is recommended. Staff met with Mr. Richard Harmon, ORC for the facility and Tim Seacrest of Equipment and Supply. A copy of the recent permit was onsite and able to be reviewed. The three most recent quarterly Chronic Toxicity samples were reviewed: All toxicity tested were at passing levels. Two months of lab data was also reviewed and there is evidence that the constituent levels are being reduced for the CVOCs being treated. The airstripper has to be manually cleaned on a monthly basis due to insitu Calcium which creates a build up in the system. RW-1 well was also reviewed during the site visit. The RW appeared to be in good condition and no issues were noted. During the site visit, it was noted that there are two missing couplings that need to be put into place from a recent system pump motor replacement. The effluent flow meter was recently calibrated and a calibration sticker was present on the meter. The flow meter calibration record was also provided for review during the site visit. Outfall-1 was not discharging during the inspection event. However, previous inspections have not indicated issues with solids, foam or discoloration being present at this outfall. All other site outfalls are related to stormwater and no flow was occuring at these outfalls. Operator logs were reviewed during the site visit and all paperwork appeared to be in good order. Prior to the site visit MRO staff made mention of a missing NDMR for July of 2015 for which an NOV had been issued. MRO staff recieved a copy of the missing NDMR during the site visit. The NOV was discovered to have been placed into the file. The ORC was unaware of the violation being issued. The ORC requests that future notification also be sent directly to him. The NOV occurred during a time of transition at Equipment and Supply. A new person is now handling the Health and Safety conditions of the facility for Equipment and Supply and is working with the ORC to maintaion the facility. V A N D E R Vr A A R T JAY ZIMMERMAN The U,S, Environmental Protection Agency (EPA) recently published the National Pollutant Discharge Elimination System (NPDES) Electronic Reporting Rule, The rule requires NPDES regulated facilities to report information electronically, instead of filing written paper reports, The rule does not change what information is required from facilities. It only changes the method by which information is provided (Le., electronic rather than paper -based). EPA is phasing in the requirements oaf the rule over a 5-year period. The two phases of the rule, and their key milestones, are: Phase 1—Starting on December21, 2016, regulated entities that are required to submit Discharge Monitoring Reports (DMRs) will begin submitting these reports electronically. If you are currently reporting your DMR data electronically using eDMR, then you simply need to continue reporting in the same way as you are now, The key change is that, starting on December 21, 2016,. electronic reporting of DMRs will be required, instead of voluntary. Phase 2 Starting on December 21, 2020, regulated entities that are required to subrnit certain other NPDES reports will begin submitting these reports electronically, Reports covered in the second phase include Notices of Intent to discharge in compliance with an NPDES general permit, Sewer Overflow/Bypass Event Reports, and a number of other NPDES program reports. Incorporating Electronic Reporting Requirements into NPDES Permits The NPDES Electronic Reporting Rule requires authorized NPDES programs to incorporate electronic reporting requirements into NPDES permits beginning December 21, 2015. Under the new rule, the electronic reporting process supersedes the paper reporting process. According to our files, your NPDES permit became effective after November 2013, and should contain the requirement to electronically report your Discharge Monitoring Reports using NC DWR's eDMR system. Permittees are required to electronically submit the required NPDES information to the appropriate initial recipient, as determined by EPA. By July 18, 2016, EPA must identify and publish on its web site and in the Federal Register a listing of initial recipients by state and by NPDES data group. Once available, you can use EPA's web site to find out or determine the initial recipient of your electronic submission. NC DWR has submitted a request to EPA to be the initial recipient for the following NPDES data groups, 1. Discharge Monitoring Reports; 2, General Permit Reports [Notices of Intent to discharge (NOIs); Notices of Termination (NOTs)]; 3. Pretreatment Program Reports; and 4. Prior to initiating groundwater sampling activities, on July 13, 2015, the water level was measured in these Equipment & Supply, Inc. 4507 Highway 74 West Monroe, North Carolina September 2, 2015 1 Harmon Environmental, PA 615 Bruce Thomas Road Monroe, North Carolina www.harmonenvironmental.com wells and piezometer. At the time of the sampling event, the groundwater at the site was flowing toward the two recovery wells. The groundwater samples collected during the July 2015 groundwater sampling event were analyzed by Prism Laboratories, Inc. for volatile organic compounds (VOCs) in accordance with SW-846 Method 8260B. Analytical results associated with this event indicated the presence of cis-1,2- dichloroethene, trans-1,2-dichloroethene, trichloroethene and vinyl chloride in the sample collected from monitor well MW-1 at concentrations exceeding the Groundwater Quality Standards established in North Carolina Administrative Code, Title 15A, Subchapter 2L (15A NCAC 2L). Cis-1,2-dichloroethene, trans-1,2-dichloroethene, trichloroethene, vinyl chloride and tetrachloroethene were detected in the sample from MW-1 at concentrations of 7,300 micrograms per liter (p,g/L), 210 µg/L, 14,000 µg/L, 28 µg/L and 8.9 µg/L, respectively. The Groundwater Quality for cis-1,2-dichloroethene, trans-1,2-dichloroethene, trichloroethene, vinyl chloride and tetrachloroethene are µg/L, 70 µg/L,100 µg/L, 3.0 µg/L, 0.03 µg/L and 0.7 µg/L, respectively. 1,1- Dichloroethene was also detected in the sample collected from monitor well MW-1 at a concentration of 20 µg/L. This compound was detected below the North Carolina Groundwater Quality Standard of 350 µg/L. Vinyl chloride was detected at a concentration of 0.78 µg/L in the groundwater sample collected from monitor well MW-6. The Groundwater Quality Standard for vinyl chloride is 0.03 µg/L. Trichloroethene was detected in this sample at a concentration of 2.8 µg/L, below the Groundwater Quality Standard for this compound of 3.0 µg/L. Cis-1,2-dichlorothene was detected in the sample from monitor well MW-6 at a concentration of 21 µg/L. The North Carolina Groundwater Quality Standard for cis-1,2-dichloroethene is 70 µg/L. Trans-1,2-dichloroethene was also detected in the sample collected from monitor well MW-6 at an estimated concentration of 8.7 µg/L. The North Carolina Groundwater Quality Standard for cis-1,2-dichloroethene is 70 µg/L. Trichloroethene was detected in the sample collected from monitor well MW-8 at a concentrations of 4.2 µg/L. The North Carolina Groundwater Quality Standard for trichloroethene is 3.0 µg/L. In addition to sampling the monitor wells, samples were collected from recovery well RW-1 and RW-2 during this sampling event. Trichloroethene was detected in the sample from RW-1 at a concentration of 57 µg/L. Vinyl chloride was detected in the sample from recovery well RW-1 at an estimated concentration of 1.9 µg/L. Cis-1,2-dichloroethene and trans 1,2-dichloroethene were detected in the sample from RW-1 at concentrations of 51 µg/L and 8.8 µg/L, respectively. Trichloroethene was detected in the sample collected from recovery well RW-2 at a concentration of 70 µg/L and vinyl chloride was detected in the RW-2 sample at a concentration of 1.8 µg/L. Cis-1,2- dichloroethene was detected in the RW-2 sample at a concentration of 20 µg/L and trans-1,2- dichloroethene was detected in the RW-2 sample at an estimated concentration of 0.9 µg/L. The vertical and horizontal extent of the volatile organic compound (VOC). impacted groundwater plume appears to be contained by the extraction system. At the request of the HWS of the DENR in a November 30, 2009 letter, Figure 11, Figure 12, Figure 13, Figure 14 and Figure 15 have been included in this report. These figures depict the detected concentrations of trichloroethene, cis-1,2-dichloroethene, trans-1,2-dichlorethene, vinyl chloride and 1,1,-dichloroethene in samples collected from monitor wells MW-1, MW-6 and MW-8 over time. As indicated in these figures, the constituent concentrations detected in the on -site monitor wells have reduced substantially since the initiation of the remediation system in July 2006. Based on these analytical results Harmon Environmental, PA recommends the groundwater monitoring and remediation activities at the Equipment & Supply, Inc. facility continue. In the March 11, 2010 letter, the DENR requested copies of the monthly influent/effluent results. The National Pollution Discharge Elimination System (NPDES) Permit issued to the Equipment & Supply, Inc. facility does not require the collection of influent samples. As a result, we are unable to provide the requested information. A subsequent September 10, 2010 letter, the DENR requested the following brief summary of the results of the routine sampling associated with the NPDES Permit re- issued on February 3, 2010. Equipment & Supply, Inc. is required to sample the treatment system effluent once a month for VOCs. This sample is analyzed by EPA Method 624 for: 1,1,2-trichoroethane;1,1-dchloroethene; cis-1,2-dichloroethene; tetrachloroethene; trans-1,2-dichlorothene; trichloroethene and; vinyl chloride. On July 6, 2015, Harmon Environmental, PA personnel collected the monthly effluent sample from the groundwater remediation system. The analytical results associated with this sample did not indicate the presence of targeted constituents at concentration exceeding the analytical method detection limit. Based on the analytical data associated with the groundwater samples collected from recovery well RW-1 and RW-2 relative to the effluent concentration detected in the sample collected from the system outfall on July 13, 2015, the groundwater remediation system is operating with an efficiency of greater than 99%. The NPDES Permit further requires Equipment & Supply, Inc. to collect a Chronic Toxicity grab sample of the treatment system effluent quarterly. Since the re -issuance of the Permit, these composite samples have passed the Chronic Toxicity test. In a November 5, 2012 1etter from the DENR, the sampling schedule was revised to annually and the DENR requested Equipment & Supply Inc. summarize the operational data (dates, times, flow meter readings pump run times, cycle data, etc.) for each of the two recovery wells. While this data is not available through the programmable logic controller, we have summarized the data accumulated during routine inspections since July 2014 Groundwater Sampling Report (Attachment D). Equipment & Supply, Inc. 4507 Highway 74 West Monroe, North Carolina September 2, 2015 3 Harmon Environmental, PA 615 Bruce Thomas Road Monroe, North Carolina www. harmonenvironmental. com 3.0 Introduction Equipment & Supply, Inc. owns and operates an aviation equipment manufacturing facility located at 4507 Highway 74 West in Monroe, North Carolina. Figure 1 depicts the location of the site on an excerpt of a United States Geologic Survey (USGS) 7.5 minute topographic map of the region. Equipment & Supply, Inc. is located on a 43 acre tract located on the southwest side of US Highway 74, within the City of Monroe, in Union County, North Carolina. The Equipment & Supply, Inc. facility occupies approximately 17 acres of the property. Equipment & Supply, Inc. began operations at the site in 1971. The main facility and associated out -buildings have been utilized exclusively to warehouse, restore and fabricate specialty metal parts for the aerospace industry. The four primary buildings on the property include an approximately 38,000 square -foot office and manufacturing facility, an approximately 10,800 square -foot concrete paneled warehouse building, an approximately 16,800 square -foot corrugated metal building comprised of two equipment assembly buildings, the former plating building and an approximately 32,000 square -foot warehouse building. Lawn areas, employee and visitor parking lots, and shipping and receiving driveways are located near these buildings. An exterior parts storage area is located southwest of the buildings. The layout of the facility is presented in Figure 2. On August 23, 1991, Equipment & Supply, Inc. received a letter from the North Carolina Department of Environment, Health and Natural Resources (DEHNR) indicating that an investigation was conducted on August 6, 1991 concerning an unpermitted discharge of overflow rinse water from a cadmium electroplating operation. Between April 1992 and September 1994 Equipment & Supply, Inc. retained an environmental consultant to assess, excavate and remove cadmium -impacted soil from this discharge area. In conjunction with these activities, five monitor wells were installed at the site. The analytical results associated with samples collected from these monitor wells indicated the presence of chlorinated VOCs in the shallow groundwater at one location on -site. In January, 1995, Equipment & Supply, Inc. submitted a Closure Plan summarizing the excavation and appropriate disposal of the remaining cadmium -impacted soil within the ditch. In additional to preparing the Closure Plan, a Groundwater Assessment Plan was submitted to the North Carolina Department of Environment and Natural Resources (DENR). Following completion of the DENR approved soil remediation activities and completion of the groundwater assessment activities, Equipment & Supply, Inc. entered into an Administrative Agreement with the DENR which required the remediation of the VOC impacted -groundwater detected at the site. In January 2006, groundwater remediation activities began at the Equipment & Supply, Inc. facility. The air stream is exhausted to the atmosphere and the .treated groundwater is discharged via a National Pollution Discharge Elimination System (NPDES) permitted outfall to an unnamed tributary of Stewarts Creek located on -site. Equipment & Supply, Inc. is required to maintain the existing monitor and recovery well network and to collect groundwater samples semi-annually. Groundwater sampling activities are conducted in general accordance with the January 16, 2008 Revised Sampling and Analysis Plan, modified to address observations presented by the Hazardous Waste Section (HWS) of the North Carolina Department of Environment and Natural Resources (DENR) December 6, 2006 and December 2, 2008, November 30, 2009 and March 11, 2010 letters and a May 24, 2010 e-mail from Mr. Adam Ulishney of the DENR to Harmon Environmental, PA. In a November 5, 2012 letter, the DENR requested Equipment & Supply, Inc. submit a .revised Sampling and Analysis Plan for the facility. The DENR requested the sampling schedule be revised from semi-annual to annual; samples be collected from the two recovery wells; summarize the operational data from the two recovery wells and; collected dissolved oxygen measurements during the July 2013 sampling event. On July 13, 2015, Harmon Environmental, PA personnel collected groundwater samples from seven on -site and off site shallow and deep monitor wells, two recovery wells and one piezometer (MW-1, MW-1A, MW-2, MW-5, MW-6, MW-8, MW-9, RW-1, RW-2 and PZ-1) for VOCs analyses. A detailed discussion summarizing these activities is presented in the following Sections. 4.0 Description of the History of the Remediation System In January 2006, groundwater remediation activities began at the Equipment & Supply, Inc. facility. VOC-impacted groundwater is pumped from two on -site recovery wells and transferred to an on -site groundwater remediation system where air stripping technology is employed to remove the VOCs. The air stream is exhausted to the atmosphere and the treated groundwater is discharged via. a NPDES permitted outfall to an unnamed tributary of Stewarts Creek located on -site. A Remediation System Start-up Report was submitted to the DENR on May 5, 2006 summarizing the corrective action system. The groundwater recovery system consists of two 75 foot deep, 6-inch diameter, PVC casing and manufactured 0.01-inch slotted screen. Each well is equipped with a Durham-GEO (Model No. TR- 2313) stainless steel well pump. The pumps were installed with the intakes located at approximately 50 feet below grade in each well. The discharge pipes and electrical supply are routed through a sanitary seal at each well head and routed below ground to the groundwater treatment facility. The groundwater treatment system consists of a 1,000-gallon equalization tank, a 0.5 horsepower (HP) transfer pump, a Product Recovery Management Inc. (PRM) Skid Mounted air stripper (Model No. 234), a discharge pump, a control panel and associated valves, gauges and flow meters. The system pumps approximately 2,800 gallons of groundwater a day. The effluent is sampled monthly and the results are submitted to the DENR in accordance with the facility's NPDES Permit. The current version of this Permit was issued to Equipment & Supply, Inc. on January 31, 2019. As previously discussed, Equipment & Supply, Inc. is required to sample the treatment system effluent once a month for VOCs. This sample is analyzed by EPA Method 624 for: 1,1,2- trichoroethane; 1,1-dichloroethene; cis-1,2-dichloroethene; tetrachloroethene; trans-1,2- dichlorothene; trichloroethene and; vinyl chloride. The results associated with the monthly samples collected since issuance on the Permit have not indicated the presence of targeted compounds in excess of the method detection limits. The NPDES Permit further requires Equipment & Supply, Inc_ to collect a Chronic Toxicity grab sample of the treatment system effluent quarterly. Since the re -issuance of the Permit, these composite samples have passed the Chronic Toxicity test. 5.0 Summary of the 2015 Groundwater Sampling Activities On July 13, 2015, Harmon Environmental, PA personnel collected groundwater samples from the seven monitor wells (MW-1, MW-1 A, MW-2, MW-5, MW-6, MW-8 and MW-9), the two recovery wells (RW-1 and RW-2) and the one piezometer (PZ-1) at the Equipment & Supply, Inc. facility. Prior to initiating groundwater sampling activities on July 13, 2015, the water level was measured in these wells and piezometer. The measured water level, and well construction information is summarized in Table 1. Following collection of the depth -to -groundwater measurements, with the exception of monitor well MW-1A, each monitor well was purged in general accordance with the procedures outlined in the March 1, 2013 Revised Sampling and Analysis Plan. The monitor wells and piezometer were purged of three well volumes or until dry using dedicated disposable Teflon® bailers. Monitor well MW lA was purged of approximately one well volume using a submersible well pump. Following completion of the purging process, on July 13, 2015, monitor wells MW-1, MW-2, MW- 5, MW-6, MW-8, MW-9 and piezometer PZ-1 were each sampled with dedicated disposable Teflon? bailers. Monitor well MW-1A was samples using the submersible pump, and recovery wells RW-1 and RW-2 were sampled via the sampling taps within the remediation system. The groundwater samples were collected directly into glassware provided by the analyzing laboratory and placed in a cooler on ice pending delivery to Prism Laboratories, Inc. (Prism) for analyses. Chain -of - Custody protocol was maintained throughout the sample shipment process. In addition to collecting representative samples for laboratory analyses, the pH and temperature of the groundwater samples from each well were measured in the field and recorded. These measurements are presentedin Table 2. Copies of the Groundwater Collection Certification Form and the Groundwater Sample Collection Log Forms are presented in Attachment A. As indicated in these field logs the field testing equipment was calibrated at 08:05 on July 13, 2015. The analytical results associated with the groundwater samples are summarized in Table 3 and Figure 7. 6.0 History of the Groundwater Sampling Activities Between 1992 and 2001 eleven monitor wells (MW-1, MW-1A, MW-2, MW-3, MW-3i, MW-4, MW-5, MW-6, MW-7, MW-8 and MW-9) and five piezometers (PZ-1, PZ-2, PZ-3, PZ-4 and PZ-5) were installed at the Equipment & Supply, Inc. facility. Historically, the on -site monitor wells have routinely been sampled in accordance with the facility's Sampling and Analysis Plan. The monitor well network was sampled quarterly until late 1995. The schedule was then modified to semi-annually sampling. Currently, monitor wells MW-1, MW-1A, MW-2, MW-5, MW-6, MW-8, MW-9 and piezometer PZ-1 are included in the routine groundwater sampling activities at the site. Well Construction Records for these seven wells and one piezometer are presented in Attachment B. No other monitor wells or recovery wells remain at the site. Monitor wells MW-3, MW-3i, MW-4, MW-7, PZ-2, PZ-3, PZ-4 and PZ-5 have been appropriately abandoned. Well Abandonment Records for these four wells and four piezometers are also presented in Attachment B. Tables 5 through 13 compare the recent groundwater sample results with historic data associated with each respective well. The laboratory data reports and Chain -of -Custody Records for the July 15, 2015 sampling activities are presented in Attachment C. 7.0 Site Hydrogeology Considerable geologic and hydrogeologic information has been generated during the soil and groundwater quality investigations conducted at the Equipment & Supply, Inc. facility. The data acquired during these assessment activities indicate the site is located within the Carolina Slate Belt of the Piedmont Physiographic Province. The Piedmont Province is a northeast -southwest trending region extending from the Hudson River in New York State to Alabama. The topography of the Piedmont is typically described as a slightly elevated region with low to moderate relief, generally dissected with valleys created by streams flowing on rocks of varying erosional resistance. Ridges and uplands are typically developed by slower weathering processes on areas of more resistant rock. Most rocks types in the Piedmont are covered with thick residual soil; however, thin soil zones are not uncommon. Soils found in the Piedmont are generally developed through in -situ weathering of fractured and faulty rock structures. The Carolina Slate Belt is generally described as consisting of low-grade metamorphosed volcanic and sedimentary rocks, situated in the east -central portion of the Piedmont Province in North Carolina. Coastal plain sediments overlap the Carolina SIate Belt to the east and the Charlotte Belt, consisting of higher grade metamorphic rocks border the Slate Belt to the west. Equipment & Supply, Inc. 4507 Highway 74 West Monroe, North Carolina September 2, 2015 7 Harmon Environmental, PA 615 Bruce Thomas Road Monroe, North Carolina www.harmonenvironinental.com Rocks comprising the Carolina Slate Belt consist of laiinated and non-Iaminated metamorphosed peltic rocks. Rock color varies from bluish -gray when fresh to brown and reddish -orange when weathered. When present, laminated bedding planes are typically well developed and exhibit bedding plane cleavage. Igneous intrusions have been observed throughout the Carolina Slate Belt. These intrusions are typically comprised of diabase and metagabbro. The diabase occurs most commonly as dikes, with a general mineralogy of plagioclase, clineopyroxene, and chlorite, with biotite and quartz filling voids. The metagabbro is generally xenoblastic, but abundant sericite and epidote obscure structure (Randazzo, 1972). Quartz veins are present throughout the Carolina Slate Belt. These veins (i.e. dikes and sills) are typically intruded into fractures in the country rock. Large muscovite flakes are commonly seen along these intrusions, possibly indicating recrystalization of the country rock at the time of intrusion. The quartz is usually milky in appearance (Randazzo, 1972). The Gold Hill Shear Zone is a major structural feature present along the western side of the Carolina Slate Belt. The Gold Hill Shear Zone is thought to be a thrust fault generally trending approximately 15 degrees east and extending southward from Southmont, in Davidson County, North Carolina to the eastern edge of Indian Trail, North Carolina. Previous investigations indicate the rocks along the western margin of the Shear Zone are characterized by higher ranking metamorphism including the occurrence of both slate and phyllite. Local fault planes, quartz veins and minor joints are also commonly associated with this major shear zone trends (Randazzo, 1972; The Geologic Map of North Carolina, 1985). Boring logs have been prepared in conjunction with the installation of monitor wells and piezometers. These logs were prepared by previous consultants during assessment projects at the site. Based on these boring logs, the Ethology underlying the Equipment & Supply, Inc. facility appears to be relatively uniform. In general, approximately six inches of top soil was present across a majority of the site. Underlying this top soil was a yellowish to orangeish brown clayey silt to silty clay material apparently weathered in -place from the underlying parent rock. This clayey silt horizon ranged in thickness from less than one foot to 10.5 feet across the site. Underlying this saprolite material was a layer of greenish gray partially weathered phyllite rock. This material ranged in thickness from one to 12 feet. The contact between the partially weathered rock and the underlying phyllite was observed to range in depth from approximately two feet below grade in the vicinity of monitor well MW-9 to greater than 25 feet in the vicinity of monitor well MW-3. The phyIlite parent rock on -site has been described as a dense dry green to gray argillite or phyllite. The aquifer underlying the site is typical of the composite weathered fractured rock aquifers located within the Piedmont of North Carolina. The aquifer is unconfined, existing under phreatic, or water table conditions. Under these conditions, the water table surface is in equilibrium with the atmospheric pressure and is not confined by low -permeable layers between the surface of the water table and the surface of the ground. Under normal conditions, the average depth to groundwater at the Equipment & Supply, Inc. is approximately 5.5 feet below the ground surface. The water flows through the pore space in the geologic media to its ultimate discharge in topographic low regions where the water table surface intersects the surface of the ground. As a result of these typical flow patterns, the potentiometric (water table) surface expressed by water table aquifers typically appears as a subdued replica of the topography. Figure 4 presents a potentiometric map of the water table at the Equipment & Supply Inc. site based on the water level measurements collected on July 13, 2015. This map represents the typical shallow groundwater flow at the site while the recovery wells are in operation. As indicated by this Figure, the shallow groundwater flow on -site is toward the recovery wells. Based on the July 13, 2015 water level data, the calculated horizontal component of hydraulic gradient ranged from approximately 0.19 feet per foot (ft/ft) to approximately 0.2 ft/ft. Based on the July 13, 2015 water level measurements, the vertical component of hydraulic gradient was calculated to be approximately 0.06 ft/ft downward in the vicinity of the monitor well MW-1IMW- lA (See Figures 5 and 6). Table 4 presents the historic groundwater elevations measured in select monitor wells since the initiation of groundwater remediation at the site. To evaluate the performance of the recovery wells and to further characterize the aquifer at the Equipment & Supply, Inc. facility, on May 15, 2006 and May 16, 2006 Harmon Environmental, PA personnel conducted a constant draw -down test on recovery wells R-1 and R-2 at the site. Recovery well R-1 was pumped at an average rate of 2.98 gallons per minute (GPM). Recovery well R-2 was pumped at an average rate of 3.9 GPM. During this test, water levels were routinely measured in recovery wells R-1 and R-2; monitor wells MW-1, MW-1A, MW-2, MW-5, MW-6, MW-7, MW-8 and MW-9; and piezometers PZ-1 and PZ-3. Harmon Environmental, PA utilized AQTESOLVTM which incorporates these assumptions to interpret the data generated during the pump test using the following three specific analytical methods: Cooper -Jacob Straight -Line Method (Cooper and Jacob, I946): This method was developed to analyze unsteady state conditions in confined aquifers. Data modifications were made to correct for the partial penetration of the wells and the decreasing saturated thickness associated with the draw - down of the water table (Kruseman, 1990). Theis Method (Theis, 1953): This method was originally developed for the analyses of unsteady -state conditions in confined aquifers. Data corrections were made to correct for the partial penetration of the wells and the decreasing saturated thickness associated with the draw -down of the water table (Kruseman, 1990). Neuman Method (Neuman, 1975): This method allows for the analysis of unsteady -state conditions from anisotropic unconfined aquifers considering delayed yield and taking .into account partial penetration of the extraction well and observation wells (Kruseman 1990). The arithmetic mean values generated by the Theis, Cooper -Jacob and Neuman Methods for wells associated with recovery well R-1 were 0.50 feet per day (ft/d), 0.52 ftld and 0.36 ftld, respectfully. The geometric mean values generated by the Theis, Cooper -Jacob and Neuman Methods for wells associated with recovery well R-1 were 0.22 ft/d, 0.29 ft/d and 0.19 ft/d, respectfully. The arithmetic mean values generated by the Theis, Cooper -Jacob and Neuman Methods for wells associated with recovery well R-2 were 1.08 ft/d, 1.05 ftld and 1.05 ft/d respectfully. The geometric mean values generated by the Theis, Cooper -Jacob and Neuman Methods for wells associated with recovery well R-2 were 0.8 ftld, 0.77 ft/d and 0.43 ftld, respectfully. r 8.0 Analytical Results As indicated in Table 2, VOCs were detected in monitor wells MW-1, MW-1A, MW-2, MW-6 and MW-8. Analytical results associated with this event indicated the presence of cis-1,2- dichloroethene, trans-1,2-dichloroethene, trichloroethene and vinyl chloride in the sample collected from monitor well MW-1 at concentrations exceeding the Groundwater Quality Standards established in 15A NCAC 2L. Cis-1,2-dichloroethene, trans-1,2-dichloroethene, trichloroethene, vinyl chloride and tetrachloroethene were detected in the sample from MW-1 at concentrations of 7,300 µg/L, 210 µg/L, 14,000 µg/L, 28 µg/L and 8.9 µg/L, respectively. The Groundwater Quality for cis-1,2-dichloroethene, trans-1,2-dichloroethene, trichloroethene, vinyl chloride and tetrachloroethene are µg/L, 70 µg/L, 100 µg/L, 3.0 µg/L, 0.03 µg/L and 0.7 µg/L, respectively. 1,1- Dichloroethene was also detected in the sample collected from monitor well MW-1 at a concentration of 20 µg/L. This compound was detected below the North Carolina Groundwater Quality Standard of 350 µg/L. Vinyl chloride was detected at a concentration of 0.78 µg/L in the groundwater sample collected from monitor well MW-6. The Groundwater Quality Standard for vinyl chloride is 0.03 µg/L. Trichloroethene was detected in this sample at a concentration of 2.8 µg/L, below the Groundwater Quality Standard for this compound of 3.0 µg/L. Cis-1,2-dichlorothene was detected in the sample from monitor well MW-6 at a concentration of 21 µg/L. The North Carolina Groundwater Quality Standard for cis-1,2-dichloroethene is 70 µg/L. Trans-1,2-dichloroethene was also detected in the sample collected from monitor well MW-6 at an estimated concentration of 8.7 µg/L. The North Carolina Groundwater Quality Standard for cis-1,2-dichloroethene is 70 µg/L. Trichloroethene was detected in the sample collected from monitor well MW-8 at a concentrations of 4.2 µg/L. The North Carolina Groundwater Quality Standard for trichloroethene is 3.0 µg/L. In addition to sampling the monitor wells, samples were collected from recovery well RW-1 and RW-2 during this sampling event. Trichloroethene was detected in the sample from RW-1 at a concentration of 57 µg/L. Vinyl chloride was detected in the sample from recovery well RW-1 at an estimated concentration of 1.9 µg/L. Trichloroethene was detected in the sample collected from recovery well RW-2 at a concentration of 70 µg/L and vinyl chloride was detected in the RW-2 sample at a concentration of 1.8 ug/L. Cis-1,2- dichloroethene was detected in the RW-2 sample at a concentration of 20 µg/L and trans-1,2- dichloroethene was detected in the RW-2 sample at an estimated concentration of 0,9 µg/L. Copies of the analytical report for the July 2015 sampling activities are presented in Attachment C. At the request of the HWS of the DENR, in a December 6, 2006 letter, Figures 8, 9 and 10 present the isoconcentration contour maps for trichloroethene, cis-1,2-dichloroethene and vinyl chloride, respectively. 9.0 Conclusions On July 13, 2015, Harmon Environmental, PA personnel collected groundwater samples from the seven monitor wells (MW-1, MW-1A, MW-2, MW-5, MW-6, MW-8 and MW-9), two recovery wells (RW-1 and RW-2) and the one piezometer (PZ-1) at the Equipment & Supply, Inc. facility in accordance with The March 1, 2013 Revised Groundwater Sampling and Analysis Plan. Prior to initiating groundwater sampling activities, the water level was measured in these wells and piezometer. As indicated in Figures 4 through 6, at the time of the sampling event, the groundwater at the site is flowing toward the two recovery wells. Analytical results associated with this event indicated the presence of cis-1,2-dichloroethene, trans- 1,2-dichloroethene, trichloroethene and vinyl chloride in the sample collected from monitor well MW-1 at concentrations exceeding the Groundwater Quality Standards established in 15A NCAC 2L. Cis-1,2-dichloroethene, trans-1,2-dichloroethene, trichloroethene, vinyl chloride and tetrachloroethene were detected in the sample from MW-1 at concentrations of 7,300 µg/L, 210 µg/L, 14,000 µg/L, 28 µg/L and 8.9 µg/L, respectively. The Groundwater Quality for cis-1,2- dichloroethene, trans-1,2-dichloroethene, trichloroethene, vinyl chloride and tetrachloroethene are µg/L, 70 µg/L, 100 µgfL, 3.0 µg/L, 0.03 µg/L and 0.7 µg/L, respectively. 1,1-Dichloroethene was also detected in the sample collected from monitor well MW-1 at a concentration of 20 µg/L. The North Carolina Groundwater Quality Standard for cis-1,2-dichloroethene is 70 µg/L. Trichloroethene was detected in the sample collected from monitor well MW-8 at a concentrations of 4.2 µg/L. The North Carolina Groundwater Quality Standard for trichloroethene is 3.0 µg/L. Cis-1,2-dichloroethene and trans 1,2-dichloroethene were detected in the sample from RW-1 at concentrations of 51 µg/L and 8.8 p.g/L, respectively. Trichloroethene was detected in the sample collected from recovery well RW-2 at a concentration of 70 p.g/L and vinyl chloride was detected in the RW-2 sample at a concentration of 1.8 µg/L. Cis-1,2- dichloroethene was detected in the RW-2 sample at a concentration of 20 p.g/L and trans-1,2- dichloroethene was detected in the RW-2 sample at an estimated concentration of 0.9 µg/L. In the March 11, 2010 letter, the DENR requested copies of the monthly influent/effluent results. The National Pollution Discharge Elimination System (NPDES) Permit issued to the Equipment & Supply, Inc. facility does not require the collection of influent samples. As a result, we are unable to provide the requested information. Equipment & Supply, Inc. is required to sample the treatment system effluent once a month for VOCs. This sample is analyzed by EPA Method 624 for: 1,1,2- trichoroethane; 1,1-dichloroethene; cis-1,2-dichloroethene; tetrachloroethene; trans-1,2- dichlorothene; trichloroethene and; vinyl chloride. The results associated with the monthly samples collected since issuance on the Permit have not indicated the presence of targeted compounds in excess of the method detection limits. The NPDES Permit further requires Equipment & Supply, Inc. to collect a Chronic Toxicity grab sample of the treatment system effluent quarterly. Since the re -issuance of the Permit, these composite samples have passed the Chronic Toxicity test. On July 6, 2015, Hannon Environmental, PA personnel collected the monthly effluent sample from the groundwater remediation system. The analytical results associated with this sample did not indicate the presence of targeted constituents at concentration exceeding the analytical method detection limit. Based on the analytical data associated with the groundwater samples collected from recovery well RW-1 and RW-2 relative to the effluent concentration detected in the sample collected from the system outfall on July 13, 2015, the groundwater remediation system is operating with an efficiency of greater than 99%. Copies of the analytical report and Chain -of -Custody Records for the July 13, 2015 sampling activities are presented in Attachment C. At the request of the Hazardous Waste Section (HWS) of the North Carolina Department of Environment and Natural Resources (DENR), in a December 6, 2006 letter, Figures 8 and 9 present the isoconcentration contour maps for trichloroethene and cis- 1,2-dichloroethene, respectively. The vertical and horizontal extent of the volatile organic compound (VOC) impacted groundwater plume appears to be contained by the extraction system. Based on the analytical results Hannon Environmental, PA recommends the groundwater monitoring and remediation activities at the Equipment & Supply, Inc. facility continue. Upon arriving at the site at 9:45 AM on September 5, 2007, Harmon Environmental, PA personnel observed the groundwater remediation system was shut down. The secondary containment dike was full of water and the ground outside the building was wet over an area approximately 170 feet long by approximately 7 feet wide.. Harmon Environmental, PA immediately reported this incident to Mr. Cile:nn Hudson with Equipment & Supply, Inc., the Moorsville Regional Office of the North Carolina Department of Environment and Natural Resources (DENR) and Union County Emergency Management. Based on our subsequent inspection ofthe system w°e have determined that the incident resulted from a faulty float switch located in the "high -high" position in the 1,000-gallon influent tank. Water accumulated in the influent tank as a result of the effluent pump inadvertently being turned off during a routine inspection. Our records indicate that on August 28, 2007, the ORC visited the site for the weekly inspection. During this inspection, the anti -siphon valve on the discharge line appeared to be fouled. The ORC shut down the effluent discharge pump to clean and repair the anti -siphon valve. Based on the position ofthe switches observed in the control panel on September 5, 2007, the ORC apparently did not reactivate the effluent discharge pump following the repair of the anti -siphon valve. Based on the hour meters, flow meters and cycle counters, it appears the effluent tank located at the base of the air stripping unit reached high alarm conditions after approximately 0.1 hours. Upon reaching high alarm, the float switch within this tank terminated the transfer of additional water into Mr. Mike Parker September 10, 2007 Page 2 the stripping tower by shutting down the Equalization Pump. The two recovery wells, GW-1 and GW-2 continued to pump filling the influent tank. The "high -high" float switch in the influent tank is designed to deactivate the two recovery well pumps when this tank is full. Inspection of this float switch after the incident indicated that this float switch was stuck in the down position. Based on the meter readings, the influent tank overflowed and flooded the containment dike over the subsequent 2-3 days. The blower appears to have flooded and shut down at approximately 5:00 PM on August 30, 2007. Based on the influent flow meters, approximately 9,600 gallons of untreated groundwater was released over the secondary containment dike as a result of this incident. As previously indicated, the released water appears to have been absorbed by the soil on -site and did not appear to reach surface waters. Further, the area of the release is within the zone -of -influence of the groundwater remediation system. Harmon Environmental, PA is scheduled to restart the system during the week of September 10, 2007. Following restart of the system, Harmon Environmental, PA has extended the weekly inspection to include testing the "high -high" float switch in the influent tank. Further, Equipment & Supply, Inc. has requested a devise be installed in the secondary containment dike to deactivate the entire remediation system in the event water begins to accumulate. A message was lett for Mr. Rex Gle:a_son in canter to provide 24-hour notification. It is our opinion that this upset may tome been caused by calcium scaling in the air stripper.
