The North Carolina Department of Transportation (NCDOT) proposes improvements to NC Highway 12 at the Coquina Beach Road/Bodie Island Lighthouse Road intersection in Nags Head, Dare County, North Carolina (Figure 1). Below and attached are a brief description of the project, figures depicting all features, and appropriate forms. Three Oaks Engineering, Inc. (Three Oaks) staff members Nathan Howell, Byron Levan, and Trevor Hall conducted the original site investigation within the study area on July 26, 2021. Eleven potential jurisdictional wetlands were identified within the study area (Table 1; Figures 2-4). No potential streams or surface waters (ponds, basins and/or tributaries) were identified within the study area. This project is located in the Pasquotank River Basin (United States Geological Survey [USGS] Hydrologic Unit Code [HUC] 03010205). A PJD site visit occurred on August 24, 2021. The following attendees were present for the site visit: United States Army Corps of Engineers (USACE) Regulatory Specialist Kyle Barnes, North Carolina Division of Water Resources (NCDWR) Washington DOT Project Coordinator Garcy Ward, North Carolina Division of Coastal Management (NCDCM) Field Representative Greg Daisey, NCDOT Division 1 Environmental Officer Paul Williams, and NCDOT Division 1 Environmental Specialist Steve Trowell. During this site visit, two wetlands (WL and WM) were added, a portion of Wetland WC was expanded to the roadway embankment toe-of-slope/drainage ditch, and a portion of Wetland WA was retracted to follow along the inside of a roadside right-of-way berm/embankment. NCDWR Stream Identification forms and North Carolina Stream Assessment Method (NCSAM) forms were not completed due to a lack of streams in the study area. USACE Wetland Determination forms (wetland and upland) are included for each potential jurisdictional wetland. North Carolina Wetland Assessment Method (NCWAM) forms were not completed since all wetlands possessed characteristics conducive to them receiving moderate or higher functional rating values and/or mitigation ratios (Appendix B). A USACE ORM Data Entry form, USACE Jurisdictional Determination Request form, USACE PJD form, and a USACE Waters Upload spreadsheet are also included with this submittal (Appendix C).
