Dear Ms. Moore:
This letter is in reference to your permit renewal request received February 26, 2021.
In a letter dated May 17, 2021, Chloe Lloyd -
Discharge Branch asked you to provide additional information (copy of letter attached) no later than the
close of business on June 16, 2021. As of today, this information has not been received, therefore, the
Division of Water Resources must return your application as incomplete in accordance with 15A NCAC
02T .0107(e)(2).
Please be advised that operation of the subject facilities without a valid permit is a violation
of G.S. 143-215.1, and may subject you to appropriate enforcement actions in accordance with G.S.
143-215.6A-6C. Civil penalties of up to $25,000 per day per violation may be assessed for failure to
secure a valid permit.
Therefore, it is imperative that the requested additional information and associated renewal
application be submitted within 30 days of receipt of this letter. Failure to provide the requested additional
information and the associated renewal application shall result in a Notice of Violation and may include a
civil penalty assessment.

A. Application:
1. The application that was submitted lists Piedmont Custom Meats, Inc. as the permittee. However,
Chapel Road Properties, LLC is the current permittee. According to the NC Secretary of State
Business Registration website, Chapel Road Properties, LLC has been administratively dissolved.
Please correct the Permittee Information on the application and clarify the business status. Limited
Liability Companies are required to maintain their registration
Office in order to conduct business in North Carolina.
2. Sign the page of the Non-Discharge System Renewal (Form: NDSR 01-
20) (attached).
B. Site Map:
1. Provide an updated site map in accordance with 15A NCAC 02T .0105(d) that shows all associated
piping and all sources contributing to the facility, along with the following (if applicable):
i. Legend, north arrow, and scale
ii. Topographic contours on all disposal sites
iii. Soil mapping units on all disposal sites
iv. All facility-related structures and fences within the treatment, storage, and disposal areas
v. All habitable residences and places of assembly within 500 feet of all treatment, storage, and
irrigations sites
vi. Location of all wells, streams (ephemeral, intermittent, and perennial), springs, lakes, ponds,
ditches, and other surface drainage features within 500 feet of all waste treatment, storage, and
disposal site(s)
vii. Latitude and longitude coordinates of all monitoring wells (decimal degrees to 6 digits)
viii. Location and identification of major components of the waste disposal system
ix. The wetted perimeter of all irrigation fields with field names (named according to the approved
permit)
x. Location and ownership of property boundaries within 500 feet of the disposal area (including
road/rail right-of-ways)
xi. The delineation of compliance and review boundaries
xii. Distance measurements verifying all setbacks are being met
xiii. Stormwater drainage controls
xiv. 100-year floodplain
Please Note: The site map is not required to be signed or sealed by a Professional Engineer.
However, the map must indicate the source of information. For example, parcel lines taken from a
county GIS map should reference the GIS website.

C. Operation & Maintenance Plan:
1. Submit an updated Operation and Maintenance Plan in accordance with 15A NCAC 02T .0507.
The plan should include the following minimum requirements:
i. Operation of the wastewater treatment, storage and irrigation systems in sufficient detail to
show what operations are necessary for the system to function and by whom the functions
are to be conducted.
ii. Anticipated maintenance of the wastewater treatment, storage, and irrigation systems.
iii. Safety measures, including restriction of access to the site and equipment.
iv. Spill prevention provisions such as response to upsets and bypasses, including how to
control, contain, and remediate.
v. Contact information for plant personnel, emergency responders, and regulatory agencies.
D. Residuals Management Plan:
1. Submit a Residuals Management Plan in accordance with 15A NCAC 02T .0508. This plan should
include the following minimum requirements: Provide an updated solids removal schedule.
i. a detailed explanation as to how the residuals will be collected, handled, processed, stored,
and disposed;
ii. an evaluation of the residuals storage requirements for the treatment facility, based upon
the maximum anticipated residuals production rate and the ability to remove residuals;
iii. a permit for residuals management or a written commitment to the permittee of a
Department-approved residuals management program accepting the residuals that
demonstrates that the approved program has adequate capacity to accept the residuals or
that an application for approval has been submitted; and
iv. if oil, grease, grit, or screenings removal and collection is a designed unit process, a detailed
explanation as to how these materials will be collected, handled, processed, stored, and
disposed.
E. Additional Documentation:
1. This facility is non-compliant with the existing permit. Condition II.1. requires that the non-
discharge facilities shall be effectively maintained and operated at all times so that there is no
discharge to surface waters, nor any contravention of groundwater or surface water standards.
i. Please provide a description of any corrective actions which have not been completed,
along with a schedule for completing them. Updates and repairs to be addressed include:
the upgradient monitoring well, bar screen and grease trap area, lagoons, pumphouse,
distribution boxes, and spray fields.
2. Please provide additional information regarding the waste generated from the tannery located
onsite. Describe the nature of the waste and how it is disposed.
3. Please provide the revised report from Summey Engineering Associates, PLLC (SEA). SEA stated
