The check valve must be repaired/replaced within the near future to ensure the treatment facility has two operational blower/motors units. It only changes the method by which information is provided (i.e., electronic rather than paper -based). EPA is phasing, in the requirements of the rule over a 5-year period. The two phases of the rule, and their key milestones, are: Phase 1—Starting on December 21, 2016, regulated entities that are required to submit Discharge Monitoring Reports (DMRs) will begin submitting these reports electronically. If you are currently reporting your DMR data electronically using eDMR, then you simply need to continue reporting in the same way as you are now. The key change is that, starting on December 21, 2016, electronic reporting of DMRs will be required, instead of voluntary. Phase 2 —Starting on December 21, 2020, regulated entities that are required to submit certain other NPDES reports will begin submitting these reports electronically. Compliance Performance: The North Carolina Department of Environment and Natural Resources (NCDENR) regulate the Catholic Conference Center effluent discharge under the National Pollutant Discharge Elimination System (NPDES). The NCDENR issued to Catholic Conference Center a NPDES Permit that includes water quality limits and sampling and monitoring requirements. The NPDES permit requires Catholic Conference Center to test for routine wastewater parameters of the treated water leaving the wastewater facility. The monitoring frequency for these tests is set at various intervals as set forth in the permit. During the year 2012, the Nh3n limit was exceeded on April 16, 2012. The reason being for the above non-compliance is the plant has very little flow, if any, going through it during that time. The only activity taking place is cleaning in the Facility. Discharge from the Catholic Conference Center Wastewater facility is received into an unnamed tributary to Camp Creek, classified WS-III Waters in the Catawba River Basin. Annual WW Report 2008 II. Compliance Performance: The North Carolina Department. of Environment and Natural Resources (NCDENR) regulates the Catholic Conference Center effluent discharge under the National Pollutant Discharge Elimination System (NPDES). The NCDENR issued to Catholic Conference Center a NPDES Permit that includes water quality limits and sampling and monitoring requirements. The NPDES permits requires Catholic Conference Center to test for routine wastewater parameters of the treated water leaving the wastewater facility. The monitoring frequency for these tests is set at various intervals as set forth in the permit. During the period at which Statesville Analytical was responsible for the plant operations for the 2008-year a total of tests were conducted. These tests were performed on the treated wastewater as it was discharged to the creek. On February 5, 2008, the heater on the BOD incubator went out and samples were frozen. The review of the DMRs revealed the following discrepanices: the effluent fecal coliform monthly average value was not calculated properly on several occasions, the permittee's information (address, phone number) and permit expiration date were not documented on the back of the DMRs, and no verification letters from the contracted laboratory were attached to the February 08 and July 08 DMRs regarding the "LA - lab accident " transcriptions. This review has shown the subject facility to be in violation of the discharge limitations found in NPDES Permit No. NC0071447. The violations are summarized in Attachment A to this letter. Based upon the above facts, I conclude as a matter of law that the Catholic Conference Center violated the terms, conditions, or requirements of NPDES Permit No. NC0071447 and North Carolina General Statute (G.S.) 143-215.1(a)(6) in the manner and extent shown in Attachment A. A civil penalty may be assessed in accordance with the maximums established by G.S. 143-215.6A(a)(2). Since dechlorination equipment had not been installed it is likely that the residual chlorine concentrations exceeded the permit on each day during the month of February 2007. This is the first month of the lowered permit limit for NC0071447 as the permittee was previously only required to monitor the residual chlorine concentration. The permit limit was exceeded by 790-1,760 percent during the month. 2) The duration and gravity of the violation; The permit was exceeded during the entire month of February and on one day during the month of March 2007. I PERMIT: The permit authorizes the continued operation of an existing 0.020 MGD wastewater treatment system consisting of a bar screen, aeration basin, secondary clarifier with sludge return, and a chlorine contact chamber with tablet disinfection. 2005. The permit for this facility became effective on November 1, 2000 and will expire on July 31, RECORDS AND REPORTS: Records and reports consisting of monthly monitoring reports, chain of custody forms, lab analysis reports, calibration logs, and Operator -in -Responsible Charge/maintenance log were reviewed at the time of the inspection. The records were organized and well maintained. FACILITY SITE REVIEW/OPERATIONS & MAINTENANCE: All treatment units seemed to be operating properly, and the facility was well maintained. The mixed liquor appeared well mixed and adequately oxygenated Screenings are disposed at the county landfill. Process control consists of settleability tests which are done twice a month. Sludge wasting is based on settleability tests. The facility is staffed with a Grade 11 ORC. A certified back-up operator has been designated and is available when the ORC is unable to visit the facility. LABORATORY: Statesville Analytical (Certification #440) in Statesville, North Carolinaprovides analytical support. The lab was not evaluated during this inspection. The pH meter, TRC meter, and thermometer appeared to be properly calibrated: EFFLUENT/RECEIVING WATERS: The facility was not discharging at the time of the inspection. The facility discharges into an unnamed tributary to Camp Creek, which is a Class WS III water in the Catawba River Basin. The receiving stream did not appear to be negatively impacted. The outfall location was accessible and well maintained. SELF -MONITORING PROGRAM: Self -monitoring reports were reviewed for the period November 2002 through October 2003. Review of the DMRs indicated numerous TRC values in excess of 28ug/I. Be aware that a stream standard of.l7ug/1 has been established for total residual chlorine for chronic toxicity effects, with 28ug/1 set as the maximum allowable effluent concentration to protect the receiving stream against acute toxicity effects. Please maintain the total residual chlorine concentration as low as possible, while still complying with fecal coliform limits. PERMIT: The permit authorizes the continued operation of an existing 0.020 MGD wastewater treatment system consisting of a bar screen, aeration basin, secondary clarifier with sludge return, chlorine contact chamber with tablet disinfection. The permit for this facility became effective on 11/1/00 and expires on 7/31/05. RECORDS AND REPORTS: Records and reports consisting of discharge monitoring reports (DMRs), laboratory analyses, chain of custody forms, Operator -in -Responsible Charge (ORC)/maintenance log, and calibration data were reviewed on -site at the time of the inspection. The records were organized and well maintained. FACILITY SITE REVIEW/OPERATIONS & MAINTENANCE: The process units appeared to be operating properly and the surrounding grounds were well maintained. The mixed liquor appeared to be adequately oxygenated and well mixed. Screenings are disposed at the county landfill. The ORC was very knowledgeable of the treatment processes and equipment used at the facility. The process control program consists of settleability tests. This office recommends the increase in frequency of the settleability measurements to twice per month. Sludge wasting is based on the settleability tests. The facility is staffed with one Grade II ORC. A certified backup operator has been designated and is available when the ORC is unable to visit the facility. LABORATORY: Statesville Analytical (Certification # 440) of Statesville, N.C. has been contracted to provide analytical support. The laboratory was not evaluated during this inspection. The pH and TRC meters and thermometer appeared to be properly calibrated. EFFLUENT/RECEIVING STREAM: The effluent was clear with no floatable solids or foam. The facility discharges into an unnamed tributary to Camp Creek, which is a WS-III water in the Catawba River Basin. The outfall location was well maintained and accessible. The receiving stream did not appear to be negatively impacted. A review of the DMRs have indicated numerous TRC values in excess of 28,41. Be aware that a stream action level of 17gg/1 has been established for total residual chlorine for chronic toxicity effects. An action level of 28,2g/1 has been set as the maximum allowable effluent concentration to protect the receiving stream against acute toxicity effects. Please maintain the total residual chlorine concentration as low as possible, while still complying with fecal coliform limits. Catholic Conference Center WWTP Page Three SELF -MONITORING PROGRAM: Self -monitoring reports were reviewed for the period March 2001 through February 2002, inclusive. No limit violations were reported. Only one TRC value was reported for the week of March 25 through March 31, 2001. The inspection verified that the second TRC value was collected and analyzed; therefore, the ORC will resubmit an amended DMR. All monitoring frequencies were correct. All field analyses appeared to have been collected and analyzed within the required holding times and all samples appeared to be properly preserved. FLOW MEASUREMENT: Effluent flow is measured instantaneously by a bucket and stop watch. SLUDGE DISPOSAL: Sludge is removed by Robert's Septic Tank Service, Inc., of Maiden, N.C. and disposal is at the City of Newton Clarks Creek WWTP. SEWER OVERFLOW: Please be advised that pursuant to Part II, Section E of your NPDES permit, and North Carolina Administrative Code (NCAC) 15A 2B .0506 (a)(2), any failure of a collection system, pumping station or treatment facility resulting in a bypass without treatment of all or any portion of the wastewater shall be reported to the central office or the appropriate regional office (Mooresville Regional Office 704-663-1699) as soon as possible but no later than 24 hours from the time the permittee became aware of the bypass. Overflows and spills occurring outside normal business hours may also be reported to the Division's Emergency Response personnel at 800-662-7956, 800-858- 0368, or 919-733-3300. A written report shall also be provided within five (5) days of the time of the incident. The report shall contain a description of the bypass, and its cause; the period of the bypass, including exact dates and times, and if the bypass has not been corrected, the anticipated time it is expected to continue; and steps taken (or planned) to reduce, eliminate, and prevent recurrence of the similar events. Any spill that reaches surface waters (i.e. any spill that reaches any water already present in a conveyance, stream, ditch, etc...) or any spill greater than 1,000 gallons on the ground that does not reach surface waters must be reported. An adequate spill response for those spills reaching surface waters should include an evaluation downstream of the point at which the spill entered surface waters to determine if a fish kill occurred. The evaluation should also include the collection of upstream dissolved oxygen and pH measurements for background information and dissolved oxygen and pH measurements at multiple points downstream of the entry point to document any negative impact. Failure to report the bypass of collection system, pumping station or treatment facility subjects violators to penalties of up to $25,000.00 per day per violation.
