Inspection Summary:
No evidence of any recent work on site. Two small dozers and a trailer remain and evidence of equipment being moved
on and off site recently but no work. Sediment control measures are holding up although they do not appear to have
been touched in months. Site appears dormant but compliant. Will check back in a few months.

The Division of Energy, Mineral, and Land Resources received a Stormwater Management Permit Application for the
Dollar General, Belews Creek, NC, Forsyth County on June 8, 2017. We have not received the additional information
requested on June 16, 2017. You have until July 24 to respond to the request for additional information, otherwise the
plans and any other documentation that were submitted will be discarded or recycled, as appropriate.
The construction of any impervious surfaces, other than a construction entrance under an approved Sedimentation
Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject to enforcement action pursuant to NCGS 143-
215.6A.
I'll be in next week June 27`^ and then out until July 251h I'll be in July 25 through August 41' for 9 days and then again
August 28 —August 31 for 4 days. I would like to get this permit issued before retire August 31". Otherwise another
engineer may have to issue the permit.

Application for the subject project on June 8, 2017. A preliminary review of that information has determined
that the application is not complete. The following information is needed to continue the stormwater review:
1. The Application Form did not include the property owner authorization (see page 5 and 6 of 6). Submit
property owner authorization.

3: Side slopes of SCMs stabilized with vegetated cover shall be no steeper than 3:1 (horizontal to vertical).
Retaining walls, gabion walls, and other engineered surfaces may be steeper than 3:1. Steeper vegetated slopes
may be considered on a case -by -case basis if the applicant demonstrates that the soils and vegetation shall
remain stable. Side Slopes not indicated on plans. Show slope on sheet C902 Detail 2 and resubmit.

12: There shall be an O&M Plan for every project subject to this Rule. The O&M Plan shall specify all
operation and maintenance work necessary for the function of all SCM components, including the stormwater
conveyance system, perimeter of the device, inlet(s), pretreatment measures, main treatment area, outlet,
vegetation, and discharge point. The O&M plan shall specify methods to be used to maintain or restore the
SCMs to design specifications in the event of failure. O&M plans shall be signed by the owner and notarized.
Application did not include the O&M Plan. Submit O&M Plan.
Otherwise the staff review of the plans and specifications has determined that the project, as proposed, will
comply with the Stormwater Regulations set forth in Title 15A Section NCAC 2H.1000.
