Donald —lust wanted to follow-up from our discussion on the phone this afternoon.
DEMLR received your application for the subject project today, June 7, 2017. As we discussed, we cannot process it until
we receive the original signed notarized revised application & O&M Agreement. If you want to send me a pdf of this
once complete, I can at least start the review process to help get this moving forward. Please let me know if you have
any questions.
1. In Part II of the form, Project Applicant should be the company you want the permit issued to lie. Randy Marion
Chevrolet of Statesville, LLC).
2. In Part II of the form, the Owner/Signing Official must be a Member/Manager of the applicant LLC. Eric Broach is
not lists by the Secretary of State as such. Alternatively, a Member/Manager of the LLC can submit an original
signed letter giving Eric Broach signature authority for this project.
3. In Part II of the form, Status of applicant should be "applicant", not "owner" since the property owners are both
Dawn Properties, LLC and Howard Family Investments #1, LLC. When this change is made, the form should then
add spaces to enter the property owners. Member/Mangers from both property owner LLC's must also sign &
notarize it.
4. The 0&M Agreement also must be signed by a Member/Manager of the applicant (Randy Marion Chevrolet of
Statesville, LLC)
