Boone Wastewater Collection System
Dear Mr. Ward:
On September 23, 2021, Gary Hudson of the Division of Water Resources (Division),
Winston-Salem Regional Office met with Mike Trivette, Operator in Responsible Charge (ORC),
Jimmy Hicks, and William Jones to conduct a Compliance Evaluation Inspection of the above
referenced Wastewater Collection System.
This inspection consists of (1) a review of the permit and accompanying documentation
to determine compliance with permit conditions, and (2) an on -site inspection of collection
system components. The attached inspection form notes the areas that were evaluated for the
inspection, with notable findings outlined as follows.
Site Inspections
1. High Priority Lines — The US Highway 421 high priority line was inspected, and no
issues were noted.
2. Pump Stations — The Industrial Park Pump Station was inspected, and no issues were
noted.

*The permit became effective February 1, 2016 and expires January 31, 2024.
*FOG flyers are mailed once per year, included in the Annual Report, and placed on the city website.
*The Town is currently under contract with a local engineering firm to upgrade the exsisting CIP.
*Mr. Trivette keeps very good records of inspections and maintenance activities.
*An adequate Spill Response Action Plan was presented and is available to all personnel.
*The Industrial Park Pump Station was inspected, and no problems were noted.
