Kennerly, PE Kennerly Engineering & Design, Inc. 12133 N NC Hwy 150, Suite F Winston-Salem, NC 27127 NORTH CAROLINA Environmental Quality July 28, 2021 Subject: Request for Additional Information Stormwater Permit No. SW4210601 Wynnfall Subdivision — Phase One Davidson County Dear Mr. Kennerly: The Division of Energy, Mineral and Land Resources (DEMLR), received a Stormwater Management Permit Major Modification Application for the subject project on June 28, 2021. A review of that information has determined that the application is not complete. The following information is needed to continue the stormwater review: 1. The submitted stormwater narrative states "Based on our research regarding Post Construction Storm Water no permit is required." Please be aware that a permit is required for this project and that the post -construction permit will be active in perpetuity. [15A NCAC 02H .1001(3)] 2. Please complete the drainage area table on Page 3 of the SWU-101 Application, including the receiving stream information and drainage area information. [I 5A NCAC 02H .1042(2)(a)] 3. Please upload electronic copies of the plan sheets at the link in Comment 5 below. [15A NCAC 02H .1042(2)] 4. Please submit an O&M Agreement, O&M Plan, and Supplement EZ form. These forms are required for all applications, including low density. [ 15A NCAC 02H .1042(2)0)] 5. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and 1 hardcopy of other documents. The requested information should be received by this Office prior to August 27, 2021, or the application will be returned as incomplete. Please reference the State assigned project number SW4210601 on all correspondence. If you need additional time to submit the information, please submit your request for a time extension to the Division at the contact below. The request must indicate the date by which you expect to submit the required information. The Division is allowed 90 days from the receipt of a completed application to issue the permit. The construction of any impervious surfaces, other than a construction entrance under an approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject to enforcement action pursuant to NCGS 143-215.6A.
