Dear Mr. Pardo:
The Division of Energy, Mineral and Land Resources (DEMLR), received a Stormwater
Management Permit Major Modification Application for the subject project on July 29, 2021. A
review of that information has determined that the application is not complete. The following
information is needed to continue the stormwater review:
1. The submitted O&M EZ for includes only the maintenance agreement. The maintenance
plan (Sheet 2 of O&M EZ) was not submitted. Please submit this or an alternative
maintenance plan for the infiltration systems [15A NCAC 02H .1017(6)].
2. The only plan sheets submitted were the details for the infiltration systems. Site plans are
also required. Please submit signed and sealed plans that contain the following
information [I 5A NCAC 02H .1042(2)(g)] :
a. Project name, designer, and dates
b. Dimensioned project or project phase boundary with bearings and distance
c. The boundaries of all surface waters, wetlands, regulatory flood zones, protected
vegetated setbacks, and protection riparian buffers or a note on the plans that none
exist
d. Proposed contours and drainage patterns
e. Site layout showing all existing and proposed built -upon areas, except for built -
upon areas associated with single family residential lots and outparcels on
commercial developments that are undetermined at the time of project submittal
f. Subdivision lot lines, maintenance access routes and easements, utility and
drainage easements, public rights of way, and SCMs
g. The location of the stormwater collection system, including the location of the
inlets, outlets, pipes, and swales, as well as the inverts and diameters of pipes,
excluding driveway culverts.

3. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and 1 hardcopy of other
documents. Pdfs must be uploaded using the form at:

Please note that this request for additional information is in response to a preliminary review.
The requested information should be received by this Office prior to October 7, 2021, or
the application will be returned as incomplete. The return of a project will necessitate
resubmittal of all required items, including the application fee. Please reference the State
assigned project number SW6210702 on all correspondence.
If you need additional time to submit the information, please submit your request for a time
extension to the Division at the contact below. The request must indicate the date by which you
expect to submit the required information. The Division is allowed 90 days from the receipt of
a completed application to issue the permit.
The construction of any impervious surfaces, other than a construction entrance under an
approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject
to enforcement action pursuant to NCGS 143-215.6A.
