Dear Mr. Shepard:
On September 29, 2021, a site inspection was conducted for the Dubose National Services, Inc. facility located at 307
Industrial Drive, Clinton, in Sampson County, North Carolina. A copy of the Compliance Inspection Report is enclosed for
your review. Mr. Tom Wilson, Materials Manager and Ms. Tyler Miles, Environmental Compliance Manager were also
present during the inspection and their time and assistance is greatly appreciated. The site visit and file review revealed
that the subject facility is covered by NPDES Stormwater General Permit NCG030000 under Certificate of Coverage
NCG030704. Permit coverage authorizes the discharge of stormwater from the facility to receiving waters designated as
Williams Old Mill Branch, class C, Sw waters in the Cape Fear River Basin.
As a result of the inspection, the facility was found to be compliant with the conditions of the NCG030000 permit. Please
refer to the enclosed Compliance Inspection Report for additional comments and observations made during the inspection.
Please be advised that violations of the NPDES Stormwater General Permit are subject to a civil penalty
assessment of up to $25,000 per day for each violation. If you or your staff have any questions, comments, or needs

Inspection Summary:
Melissa Joyner met with Mr. Tim Shepard, Fabrications Division Manager, Tom Wilson, Materials Manager and Ms. Tyler
Miles, Environmental Compliance Manager at the Dubose National Energy Services, Inc., facility where she reviewed the
Stormwater Pollution Prevention Plan (SWPPP). The SWPPP contained the information required by General Permit
NCG030000. As a reminder, Analytical and Qualitative monitoring, as well as facility inspections are now required to be
conducted quarterly per the schedules in the General Permit. except for the following information. It is recommended on the
internal "Facility Inspections" form that verbiage be included about inspecting the ditches. The facility grounds were
inspected including the outfalls and the best management practices. It is recommended maintenance be continued with
sweeping up the dirt observed near a couple of drop inlets. Otherwise, all appeared to be good condition.
