Introduction
The City of Mount Holly is a regulated Phase II Municipal Separate Storm Sewer System (MS4)
community with a National Pollution Discharge Elimination System (NPDES) permit (NCS000407). This
Municipal Facilities Operations and Maintenance (O&M) Plan is a component of the City's approach to
comply with the permit's Pollution Prevention and Good Housekeeping (PPGH) minimum measure.
Additional information can be found in the City's Stormwater Management Plan (SWMP), and the City's
Illicit Discharge Detection and Elimination (IDDE) Program.
PPGH for municipal operations includes the following programs:
(1) Municipal Facilities Operation and Maintenance (O&M) Program
The City maintains a current inventory of municipal faciliti erforms facility inspections and
routine maintenance with established frequencies, sched and documentation; provides staff
training on general stormwater awareness and imple in GH practices; and ensures that
municipal industrial facilities subject to NPDES in rial stor ter permitting comply with
permit requirements.

(4) Municipal SCM Operations and Maintenance (O&M) Program
The City manages, operates, and maintains (2) municipally owned structural SCMs previously
installed for compliance with the post -construction program NPDES permit requirements.
The program is detailed in Part 4 of this document, including locations, construction
drawings/sketches, maintenance frequencies, and schedules for these SCMs.
(5) Pesticide, Herbicide and Fertilizer Management Program
To minimize water quality impacts from the use of landscape chemicals at City -owned facilities
and parks, City staff authorized to apply landscape chemicals must maintain a current applicator
license. Staff applicator licenses are included in Appendix G.

(6) Vehicle and Equipment Maintenance Program
The City provides routine pollution prevention training to those staff members that service,
maintain, and/or clean municipal vehicles and equipment. The designated vehicle and equipment
maintenance areas are routinely inspected. Schedules and frequencies are detailed in Part 6 of
this document.
(7) Pavement Management Program
The City performs regular street sweepings and seasonal leaf pickups, detailed in Part 7 of this
document.

1.1 Municipal Facility Inventory and Inspection Details
All City -owned facilities with the potential to generate pollution through stormwater runoff are
required to be inventoried, regularly inspected, and properly managed. Chemical storage, exposed
materials and machinery, and work yards are common municipal operations and facilities with the
potential to pollute stormwater. The City of Mount Holly owns properties falling under the
following categories:
• Industrial (No Exposure)
• Wastewater Treatment Plant
• Fire Stations
• Public Park
• Vacant Lot
• Pump Station
A list of specific City -owned facilities determined as po\sspection
butors of pollution to
stormwater runoff are presented in Table 1-A. Additio inabout these facilities,
including, maps, an assessment of polluting materials site, checklists are
included in Section 1.2. The remainder of City -ow acili 'sfied as contributors to
stormwater pollution are included Appendix A. All - ed facilities are evaluated annually
for their potential to generate stormwater pollution eet the requirements of the NPDES

2.2 Spill Response Procedures
1. RECOGNIZING A SPILL:
When a spill is discovered, first determine if you have the proper training, PPE, and supplies to
respond safely.
REPORTABLE SPILL (ANY of the following are true):
a) Spill is greater than or equal to 25 gallons
b) Spill causes a sheen on surface water
c) Spill is within 100-ft of a surface water conveyance (i.e., storm drain inlet, swale,
channel, creek, lake, river, or pond)
d) Spill reaches surface waters (i.e., creek, lake, river, or pond)
e) Spill has not been cleaned up within 24 hours
a) Spill is less than 25 gallons IV

remediation companies to remove the spill/contaminated material(s).

of the NCDEQ at (704)-663-1699 within 24 hours of the spill.

3.1 Procedures for Inlet Cleanings
The Streets & Solid Waste Department inspect storm drains and catch basins along priority streets
prior to or following heavy rain events/flash flood warnings. Storm drains and catch basins
inspections include, but are not limited to the following:

3.2 Procedures for Outfall Inspections
The Stormwater Department is responsible for conducting dry weather outfall inspections twice
per year. The City's Illicit Discharge Detection and Elimination (IDDE) Plan details procedures for
identifying potential illicit discharge. If flow is detected during routine dry weather IDDE outfall
inspections but no physical/chemical indicators suggest illicit discharge, groundwater seepage may
be occurring due to leaky infrastructure. If potentially leaky infrastructure is detected the following
is to be noted:

6.1 Daily Vehicle/Trailer Inspection Procedures
The City provides routine pollution prevention training to those staff members that service,
maintain, and/or clean municipal vehicles and equipment. The designated vehicle and equipment
maintenance areas are routinely inspected and maintained to prevent potential hazardous leaks.
The City requires all municipal vehicles to be inspected prior to and following each trip. An
inspection form is to be filled out accordingly (see Appendix H). If maintenance is required, it is to
be reported to the appropriate personnel.

7.1 Street Sweeper SOPs and Schedules
The Streets and Solid Waste Department is responsible for cleaning all City -owned streets and
parking lots according to the dedicated routes (see Leaf Pickup Routes Map in Appendix 1). Street
sweeping is performed on a weekly basis, and after large storm events. The street sweeper is also
used following outdoor events, (i.e., Springfest, Festivals, and Parades).
A record log of the dates and routes completed by the sweeper truck is provided in Appendix I.

dedicated routs.
This is done on a weekly basis however, after a storm event or inclement weather the
sweeper may be used more often.

The Streets & Solid Waste Department check storm drain tops on priority streets before and after
heavy rain events or flash flood warnings. They inspect storm drain/catch basins for build-up of
sediments, debris, trash, vegetation, structural conditions etc. If the structural condition requires
maintenance, it is reported to the Stormwater Department for inspc n and to proceed with a
repair plan. They use the leaf pick up map color coordinated wi ut numbers. The map is
updated as needed.

The Streets & Solid Waste Department check storm drain tops on priority streets before and after
heavy rain events or flash flood warnings. They inspect storm drain/catch basins for build-up of
sediments, debris, trash, vegetation, structural conditions etc. If the structural condition requires
maintenance, it is reported to the Stormwater Department for inspc n and to proceed with a
repair plan. They use the leaf pick up map color coordinated wi ut numbers. The map is
updated as needed.

The Streets & Solid Waste Department check storm drain tops on priority streets before and after
heavy rain events or flash flood warnings. They inspect storm drain/catch basins for build-up of
sediments, debris, trash, vegetation, structural conditions etc. If the structural condition requires
maintenance, it is reported to the Stormwater Department for inspc n and to proceed with a
repair plan. They use the leaf pick up map color coordinated wi ut numbers. The map is
updated as needed.

at 704-813-0251.
Responsible Party: Stormwater Specialist
Stormwater Department SOP are as follows:
Access the situation & implement proper clean up procedure by enting the following:
• Date, time, location of the spill
• Name, address & phone number of the responsible y
• Type and quantity of product spilled
• Attach pictures including date, time & descr' n

• Cause of the spill
• Description of any mitigating facto luding t ads, power failure etc.)
• Witness to incident
• Persons contacted on scene

information necessary.

Oleof the NC DEQ at 704-663-1699 for all oil spills
greater than or eq o 25 gar spi is within 100ft of a surface water conveyance, including
storm drain inlets, s chcreeks, lakes, ponds, etc. A spill of any amount that reaches
surface waters must be rtfter hours, they are to be contacted by 8AM the next business
day.
* Contact Gaston Co Nat= at 704-922-4181 for sediment & erosion concerns or fish
kills exceeding 100.
• Always perform a follow up investigation to ensure that the cleanup has been completed
in accordance with local and State regulations.
