Lightly developed rural area.Site is situated between a manufacturing facility and a church and its grounds and other facilities.Residences are nearby.Warren Wilson College within a
mile.Charles Owen District Park is downsteam.

4h.Explain the purpose of the proposed project:*

The project was conducted as the first phase to repair an undercut and void beneath and behind end bent 2 of the bridge.A 6-8'deep scour hole in front of the end bent was filled.
Phase 2 will consist of boring through the roadway or shoulders to pump in grout to completely fill the void behind the end bent.
4i.Describe the overall project in detail,including indirect impacts and the type of equipment to be used:*
Water was diverted through a diversion channel.Water that got around the diversion was pumped from the scour hole.Steel rebar was doweled into the existing end bent face.Forms
were constructed to contain the concrete until it cured.Concrete was poured between the forms and existing end bent.Some concrete flowed under the end bent cap to seal off and
partially fill the void.
Equipment included track hoe,pump,concrete pump.
