Hi, Jack,
The Dormant Status can be accomplished with submittal of a form on-line. The information is actually under the NCG02
General Permit section of our website, just under the Qualitative Monitoring Forms. I've included a screen shot below.
Use the Dormant Status Request form for these to request/re-establish dormant status. The CO will then route the
request to the appropriate RO. The annual certification form (second one down there) is used each year thereafter. I
have attached a copy of both forms for your convenience — they need to be completed and uploaded at the link on the
website.
Dormant Status
* Attach the completed NCGO2 Dormant Status Request For 2-11 to the

* Attach the ccrnpleteci NCG02 Dormant Status Annual Certification Form 0 to
the Stormwa ter U reload Farm.
D-3. Renewal of DDnUant Status
The permitteeshallself-recertiFy that Do rmant Status cond i tion s are being maintained
annually by completing a Dormant Status Certification Farm [available at
stormwater?. In this form. the permittee will certify that the facility is still temporarily
inactive and all portions or the site where clearing, grading and/or excavation activities
have occurred are stabilized with permanent uegetatton. This documentation shall be kept
with the SWPPP and available for inspection u pon request. The perrnittee mast request
reissuance of Dormant Status in writing by submitting a written request to the Division's
Central Office within thirty (30) days prier to the expiration or this General Permit to
maintain Dormant Status,

Would you help me with the following requests?
Hanson received "dormant status" for its inactive Senter Sand & Gravel Mine in 2018, and I would like to recertify for
continued reduced monitoring. Would you send me the proper form; I was unable to find it.
For our Durham Quarry, it operates as a sales yard for stone hauled in from our nearby Rougemont Quarry. The City
owns the former pit, and it uses the former pit as a water reservoir. Stormwater run-off is only affected by stockpiled
wash stone. The RRO once recommended that we rescind the permit, but Hanson has reservations about rescinding an
environmental permit. Perhaps we could apply for dormant status. I've attached the 2009 RRO stormwater inspection.
I need to make a similar request for our inactive Elm City Quarry. The attached 2015 reduced monitoring request was
never answered. A 2015 multi -media inspection is attached.
And finally, I would like to make the same request for our Lexington Quarry in Davidson County. That is a greenfield site
with no disturbance. Previous W-SRO mine and stormwater inspections are attached.
