The information is actually under the NCG02 General Permit section of our website, just under the Qualitative Monitoring Forms. I've included a screen shot below. Use the Dormant Status Request form for these to request/re-establish dormant status. The CO will then route the request to the appropriate RO. The annual certification form (second one down there) is used each year thereafter. I have attached a copy of both forms for your convenience — they need to be completed and uploaded at the link on the website. Hanson received "dormant status" for its inactive Senter Sand & Gravel Mine in 2018, and I would like to recertify for continued reduced monitoring. Would you send me the proper form; I was unable to find it. For our Durham Quarry, it operates as a sales yard for stone hauled in from our nearby Rougemont Quarry. The City owns the former pit, and it uses the former pit as a water reservoir. Stormwater run-off is only affected by stockpiled wash stone. The RRO once recommended that we rescind the permit, but Hanson has reservations about rescinding an environmental permit. Perhaps we could apply for dormant status. I've attached the 2009 RRO stormwater inspection. I need to make a similar request for our inactive Elm City Quarry. The attached 2015 reduced monitoring request was never answered. A 2015 multi -media inspection is attached. And finally, I would like to make the same request for our Lexington Quarry in Davidson County. That is a greenfield site with no disturbance. Previous W-SRO mine and stormwater inspections are attached. Interim Director Subject: Exemption from Monitoring During Shut -Down (Site Dormancy) Certificate of Coverage NCG020156 Hanson Aggregates - Senter Plant Harnett County Dear Mr. Garvey: A Compliance Evaluation Inspection performed at the Hanson Aggregates - Senter Plant in Harnett County, NC on August 23, 2018 by Mike Lawyer of the Fayetteville Regional Office of the Division of Energy, Mineral and Land Resources verified that no industrial activity was taking place at the time of the inspection. Your request to maintain coverage under the NCG020000 permit, but to discontinue stormwater and wastewater monitoring is hereby approved, based on the following conditions: There shall be no industrial activities taking place, no stockpiles of raw materials (to include chemical additives) stored on the site, or wastewater or stormwater industrial discharges while the permittee is not monitoring. No wastewater shall be generated and all wastewater treatment units shall be cleaned, stabilized, and inactivated as to render them harmless to the environment while the site is dormant. Qualitative and analytical monitoring actions as described in Part IV, Sections A, S, C, and D do not have to be conducted as long as the above conditions and all other permit conditions and limitations are met, including: o The annual permit fee must be paid. Failure to pay the annual permit fee will be considered a violation of the subject permit o The Stormwater Pollution Prevention Plan (SPPP) shall be kept updated to reflect the status of the facility. Updates and conditions of the SPPP of note are: The BMP Summary shall be reviewed and updated annually. ■ Training programs shall be developed and training provided at least once a year for facility personnel with responsibilities for: spill response and cleanup, preventative maintenance activities, and for any of the facility's operations that have the potential to contaminate stormwater runoff. ■ All aspects of the Stormwater Pollution Prevention Plan shall be reviewed and updated on an annual basis. The annual update shall include an updated list of significant spills or leaks of pollutants for the previous three years, or the notation that no spills have occurred. The annual update shall include written re -certification that the stormwater outfalls have been evaluated for the presence of non-stormwater discharges. (Should non- stormwater discharges be discovered, the permittee shall contact this office as soon as possible, but in no case greater than 24 hours or the next business day). No wastewater is generated, and there are no wastewater treatment units. There are no stormwater discharges resulting from land disturbance activities, and sources of stormwater pollution have been properly disposed off -site. Results from mine inspections conducted by your Department on 8/16/2017 and 1/25/2018 show that there is no industrial activity occurring at this site, and the site is stabilized. Due to the fact that the site is inactive, unstaffed, and is stabilized, Hanson Aggregates Southeast, LLC requests that its Senter Sand & Gravel Mine be exempt from monitoring as described in the Permit as long as the site remains dormant and stabilized. BMP Summaries will be reviewed and updated annually. Annual permit fees will be paid, and all SWPPP aspects will be reviewed and updated annually.
