The following is an abbreviated application form for those applying for a 401 Water Quality
Certification to impact FERC (Federal Energy Regulatory Commission) regulated lake shore or
bottom under the U.S. Army Corps of Engineers Permit GP30. This Application cannot be used
for wetland or stream impacts. Please provide seven (7) copies of this application and supporting
information as well as a non - refundable application fee to the Division of Water Quality of $240.00
for impacts of to lake bottom of less than 1 acre and $570.00 for impacts to lake bottom of greater
or equal to 1 acre.

2.Grade 10' Haul Road to area of Gabion Wall Construction;
3,Grade and Install Stone Foundation for Gabion Wall to Specified Elevation;
4. Install first course of Gabion Wall (9 foot wide base);
5. Maintain control of high turbidity water in construction area;
6.Complete Installation of Gabion Baskets/Stone until final height is reached;
7.13ackfill area behind Gabions with Class I Rip Rap and properly Compacted Soil;
8.Perform Final Grading and Seed and Stabilize all disturbed areas;
9.Remove Sandbags and Turbidity Curtain after Stabilization is complete.
