Laboratory
Are field parameters performed by certified personnel or laboratory?
Are all other parameters(excluding field parameters) performed by a certified lab?
# Is the facility using a contract lab?
# Is proper temperature set for sample storage (kept at less than or equal to 6.0
degrees Celsius)?
Incubator (Fecal Coliform) set to 44.5 degrees Celsius+/- 0.2 degrees?

Comment: On -site field analyses (pH, total residual chlorine, temperature) are performed under
the facility's field laboratory certification # 5479. K&W Labs (Certification #559) has
also been contracted to provide analytical support
Effluent Sampling Yes No NA NE
Is composite sampling flow proportional? ❑ El • El
Is sample collected below all treatment units? • LI El El
Is proper volume collected? • ❑ ❑ ❑
Is the tubing clean? ❑ El • ❑
# Is proper temperature set for sample storage (kept at less than or equal to 6.0 El El • El
degrees Celsius)?
Is the facility sampling performed as required by the permit (frequency, sampling type • El El El
representative)?
Comment: The subject permit requires effluent grab samples.
Operations & Maintenance Yes No NA NE
Is the plant generally clean with acceptable housekeeping? ❑ • ❑ El
Does the facility analyze process control parameters, for ex: MLSS, MCRT, Settleable DOME
Solids, pH, DO, Sludge Judge, and other that are applicable?
Comment: The facility appeared to be adequately treating the wastewater at the time of the
inspection. However, at the time of the inspection the facility showed signs of aging and
is in need maintenance. The bar screen appeared too coarse and insufficient to catch
the debris of the influent flow. Rags, paper products and other debris were observed in
downstream treatment units. A settleability test of solids at the time of the inspection
was very low settling in the 10% range after 15 minutes. Slow settling may be an
indication of insufficient wasting. At the time of the inspection, only one of the two
blowers was working the other was out of service earlier that week. It was observed that
the skimmers in the clarifier were not removing the floating solids sufficiently and the
clarifier had floating solids. At the time of the inspection, the wasting pump was not
working. Advanced signs of rusting were clearly visible in the digester and adjacent
wet well.
Bar Screens Yes No NA NE

Is the DO level acceptable? ❑ ❑ ❑ •
Is the DO level acceptable?(1.0 to 3.0 mg/I) ❑ ❑ ❑ •
Comment: Dog food is added as a food supplement five days per week (at a minimum) due to the
low influent flows/loading. The facility is equipped with two blower/motor units, however,
one of the blowers was not working at the time of the inspection.
