SUBJECT: Request for Authorization to Construct
Leith Creek WWTP Sludge Improvements
NPDES Permit No. NC0020656
The Construction Grants & Loans Section received your request for Authorization to
Construct and the supporting documentation on July 7, 2004. Prior to beginning our technical
review, the following additional information must be provided:
• A complete copy of the NPDES Permit or public notice of a draft NPDES Permit;
• The design capacity of the treatment plant in million gallons per day;
• Documentation that the Erosion and Sedimentation Control Permit application has
been submitted to the Division of Land Resources;
• A residuals management plan;
• A construction sequence or identification of the location of the sequence within the
project's plans or specifications;
• A signed/sealed certification from the Engineer -of -Record that the project
complies with N.C.G.S. 133-3.
In addition, please have your engineer complete the enclosed Authorization to Construct
application form. This additional information should be provided by August 16, 2004.
