The Division of Water Resources (DWR) received application sets and $570 fee for the above referenced project. A quick
review of the project revealed a couple of items that will need to be addressed before DWR can address this project for
review and approval.
Under item #5, Project History the 'Yes' box is checked as having had earlier permits or certifications obtained and /or
requested for this project. Please advise DWR if there were any prior Water Quality 401 approvals issued for this
property and what the DWQ number might have been.
The PCN application was not signed or dated by you. The agent authorization from M/I Homes was included in the
package, but the application requires signature and date. If you are able, please address these two items. A signed and
dated page can be scanned and emailed to me.
