This form must be completed by Permit Writers for all expedited permits which do not require
full Fact Sheets. Expedited permits are generally simple 100% domestics (e.g., schools, mobile
home parks, etc) that can be administratively renewed with minor changes, but can include
facilities with more complex issues (Special Conditions, 303(d) listed water, toxicity testing,
instream monitoring, compliance concerns).

The USDA — US Forest Service operates a WWTP with a permitted flowrate of 0.02
MGD. The facility generates a continuous discharge of wastewater from community
showers, dump station, and full hook-up sites. The WWTP utilizes:

• Manual bar screen
• Flow control splitter box
• 20,000 gallon aeration basin

• Tertiary filters

• Map Correction — the map included with the draft permit was inaccurate and did
not show the location of the facility or outfall. The map in the final permit has
been corrected.
• Stream Classification and Segment — The receiving stream segment and
classification have been corrected in the final issued permit. Previous permits had
indicated a stream segment of 6-67-(1) which is Bent Creek from its source to
Powhatan Lake dam and is a class B;Trout waterbody. However, the outfall
location for the discharge is actually located below Powhatan Lake in segment 6-
67-(7) which is still Bent Creek, but is class B and does not carry the Trout water
designation.

I just wanted to touch base about you NPDES permit renewal for Lake Powhatan Lake WWTP. The
permit has finished it's notice period and we are getting things ready for final issuance. I have taken
over finalization of the permit as the previous permit writer (Brianna Young) has taken a new job within
our Department. You may have had communications with Brianna previously about electronic
transmittal of documents. If so, I apologize for the duplication of the information below.
In order to provide more convenience, control, and security to our permittees and assist them in processing their
transactions, The Division of Water Resources is currently transitioning towards electronic correspondence. This
will hopefully provide more efficient service to our permittees and other partners and will allow us to more
effectively process and track documents. We are writing to ask you for your approval of the transmittal of
documents related to your permitting and related activities with the Division in an electronic format. Documents
will be emailed to the appropriate contact person(s) in your organization in a PDF format.

The USGS quad is Skyland. Don't know the HUC or Subbasin. Thought Bent Crk was trout. You'll need to check on the segment but I think it is 1 not 7.
And the lat/long is 35 28' 57.2"N/82 37' 21.8"W.

Since Brianna has moved on to the Stormwater Program I have been reassigned some of the renewals she was working on. This is one of
them and it is set to be issued. I think I saw something in the files to indicate this one was yours. I didn't see any response back on the draft
permit so I just wanted to make sure you were okay with this one before getting it finalized. I think Brianna sent the draft to you guys back
in July maybe.

Linda — here are a couple of maps from our stream classifications website. Could you verify the
segment? I believe the discharge outfall is below the Lake (Powhatan) which looks like it would be in
stream segment 6-67-(7) which would be class B.
