Enclosed please find a copy of the Compliance Evaluation Inspection form from the inspection conducted on
October 24, 2005. The Compliance Evaluation Inspection was conducted by Mark Brantley and Don Register of
the Fayetteville Regional Office. The facility was found to be in Compliance with permit NC0020656. The
cooperation of Mr. Robert Ellis, Grade IV ORC/Treatment Plants Director and Ricky Odum, Grade III Backup
Operator was appreciated. As a reminder preservation of the Waters of the State can only be acquired through
consistent NPDES permit compliance.

The treatment plant was overall clean and neat in appearance. It was noted during the inspecton that about 8"
of sludge was in the chlorine contact chamber. This is currently not a problem but as a reminder the
Fayetteville Regional Office requires that less than one foot of sludge be maintained in the chlorine contact
chamber at all times.

Comment: Facility analyzes MLSS, settleable solids, pH, dissolved oxygen, and the sludge blankets in the clarifiers
for process controls.

Comment: The alarm is tested everyday at the influent pump station. The facility has four influent pumps that
alternate run times. At the time of the inspection pump #3 was out of service due to a check valve that had started
leaking. Parts have been ordered to correct the problem.

Comment: Waste water is black due to an industry that dies cardboard. Clarifiers #1 and #2 are thirteen feet deep
and have a sludge depth of 2.5' and 4.5' respectively. Clarifier #3 is 14' deep and had a sludge depth of less than 1'.

Comment: Pump #1 in the recirculation pump room on the extended aeration side of the plant is out of operation for
repairs. A few spare parts are on hand and the facility uses Charles Underwood of Sanford, NC to the maintenance
work that the plant staff can not handle.
