Enclosed please find a copy of the Compliance Evaluation Inspection form from the inspection conducted on
November 14, 2006. The Compliance Evaluation Inspection was conducted by Mark Brantley, Environmental
Chemist, of the Fayetteville Regional Office. The cooperation of Mr. Ricky Odum, Grade IV Back Up ORC, was
greatly appreciated. The facility was found to be in Compliance with permit NC0020656. As a reminder,
preservation of the Waters of the State can only be achieved through consistent NPDES Permit compliance.

The facility was clean and neat at the time of the inspection.
COMMENTS •
The garbage dumpster at the grit chamber was leaking a very small amount of wastewater onto the ground.
Pleaseput the drain plug in the dumpster or install a berm like structure to ensure wastewater flows back
into the grit chamber instead of on the ground.
Alarms at the influent pump station were not tested during the inspection. A log of test dates and times of
testing the alarms are kept by plant staff.
As a safety note, please secure sulfur dioxide cylinders to prevent them from falling. Mr. Odum, back up
ORC, indicated that plans have been made to install safety chains.
DMR's for the months of February 2006,"April 2006, and July 2006 were reviewed and appeared to be
correct.
A violation report for the time period between October 2005 through August 2006 revealed a total
suspended solids violation (monthly average exceeded) and two pH violations (daily minimum not
reached). A notice of violation was issued for the three violations.
The Leith Creek WWTP analyzes a large number of the requi red NPDES parameters. Due to the large
amount of laboratory work that is done by Mr. Ricky Odum, Chief. Operator, the city may find it beneficial to
hire someone to run the required laboratory tests. This staff member would also be available to r un a
variety of process control tests that would generate data in a timely manner that would assist the plant
operators in running the plant. This would allow Mr. Ricky Odum to spend more time doing his operational
duties.

Comment: Oxford laboratories performs Total Nitrogen, Total Phosphours, Mercury,
and Toxcity tests. The BOD thermomter was reading 22 degrees celsius. Please check
the thermomoter and the incubator to make sure everything is working properly and
possibly prevent a future problem.
