application submitted on 6/2/2021 and payment received on 6/25/2021.

Also, attached is the additional information request sent on 6/25/2021. This information was requested 75 days ago and has
exceeded the 30-day response time.

Pursuant to Title 15A NCAC 02H .0502(e), the applicant shall furnish all the requested information for the proper consideration
of the application.
If all the requested information is not received within 30 calendar days of receipt of request, the Division will be unable to
approve the application and it will be denied as incomplete. The denial of this project will necessitate reapplication to the
Division for approval, including a complete application package and the appropriate fee.

Please provide either the originally requested authorization form or fill out a new application reflecting the changes to the
authorized agent by the end of business today.

I could not find the authorization I was looking for, but I did find the attached. It was for our environmental consultant and
covered the entire neighborhood. Since we have work going on through the neighborhood could we use the same format to
avoid having to provide an individual authorization for each area?

On June 2, 2021, the Division of Water Resources (Division) received your application requesting a 401 Water
Quality Certification from the Division for the subject project. An email containing instructions to submit the
required application review fee was sent on June 3, 2021. The Division has determined that your application is
incomplete and cannot be processed. The application is on-hold until all of the following information is received:

1. A signed and dated copy of the Agent Authorization letter must be submitted if the Agent has signed this
