As a result of the recent SBI inquiry that was conducted on January 18, 2007 into the allegations of the mismanagement
of wastewater residuals, it has prompted serious areas of concern by this office. According to our records you are the
Certified Grade III Back-up Operator in Responsible Charge of this facility as well as the Certified Land
Application/Residuals Operator in Responsible Charge of the wastewater residuals from the airport wastewater treatment
plant.. With this information, we must move forward with a "Show Cause Hearing". You are therefore requested to attend
the "Show Cause Hearing" on February 13, 2007, at which time the Fayetteville Staff of the Division of Water Quality,
Surface Water Protection, would like to offer you the opportunity to discuss these issues and/or produce any applicable
records for the year 2005 &.2006. This "Show Cause Hearing will take place at 10 am, in the 7th floor conference room of
the Division of Water Quality Offices located in the Systel Building, 225 Green St., Fayetteville, North Carolina.
Please respond within 5 days of receipt of this letter to acknowledge your desire to discuss this matter.
If you fail to respond or do not show for the hearing, disciplinary action will proceed.
If you have any questions concerning this matter, please do not hesitate to contact either Mr. Don Register, Wastewater
Treatment Consultant, or myself at 910-433-3326.
