Dear Mr. Beal,
Enclosed please find a copy of the Compliance Evaluation Inspection Report
for the inspection we conducted at the Town of Maiden Public Works Facility on
September 1, 2021 Please note that in the stormwater inspection report summary
we ask that the Stormwater Pollution Prevention Plan (SWPPP) needs to be updated
to comply with the current permit cycle and qualitative monitoring needs to be
completed prior to flows entering the storm sewer system.

Inspection Summary:
The permittee has completed a Stormwater Pollution Prevention Plan (SWPPP) in the past. However, this plan is outdated
and has not been reviewed. The SWPPP needs to be updated to the current permit cycle. Qualitative monitoring has not
been completed per the permit. The facility has three inlets on site that discharge directly into a stream. Currently, sampling
is done where the stream leaves the site. Qualitative monitoring samples need to be collected prior to flows reaching the
stream. Creating a weir upstream of the inlets will allow concentrated flow for sampling. The permitted has until December 3,
2021, to submit to the Mooresville Regional Office the updated SWPPP and qualitative monitoring report

Comment: The Stormwater Pollution Prevention Plan (SWPPP) was created in 2015 but has not been fully
implemented. No documentation for facility inspections, annual certifications or spills

available at the time of the inspection. The SWPPP has not been maintained since it was
created.
