Skvarla, III Governor Director Secretary August 2, 2013 Greene County DWQ Project No. 20130386 v.2 TIP B -4755 Bridge 65 MODIFICATION of APPROVAL of NEUSE RIVER BUFFER RULES (15A NCAC 2B.0233) AUTHORIZATION CERTIFICATE and NEUSE RIVER BUFFER PROTECTION RULES MINOR VARIANCE 115A NCAC 2B.0233 (9)(b)) Dr. Greg Thorpe, PhD., Manager Project Development and Environmental Analysis North Carolina Department of Transportation 1598 Mail Service Center Raleigh, North Carolina, 27699 -1598 Dear Dr Thorpe: You have our approval, in accordance with the conditions listed below, for the following impacts for the purpose of replacing Bridge Number 65 over Appletree Swamp on SR 1215 in Greene County: Buffer Impacts in the Neuse Basin This approval is only valid for the purpose and design that you submitted in your modified application dated received July 31, 2013. All the authorized activities and conditions associated with the original Buffer Authorization dated July 3, 2013 still apply except where superceded by this certification. In addition, you should acquire any other federal, state or local permits before you proceed with your project including (but not limited to) Sediment and Erosion Control, Non - Discharge and Water Supply Watershed regulations. This authorization is valid solely for the purpose and design described in your application (unless modified below). Should your project change, you must notify the DWQ and submit a new application. If the property is sold, the new owner must be given a copy of this authorization letter, and is thereby responsible for complying with all the conditions. For this authorization to remain valid, you must adhere to the following conditions. This modification is applicable only to the additional proposed activities. All of the authorized activities and conditions associated with the original Buffer Authorization dated July 3, 2013 and subsequent modifications still apply except where superceded by this authorization. 2. A copy of this modification shall be maintained on site at the construction site at all times. In addition, the original authorization and all subsequent modifications, if any, shall be maintained with the Division Engineer and the on -site project manager. This letter completes the review of the "No Practical Alternatives" determination under 15A NCAC 2B .0233. This letter does not authorize any impacts to either Waters of the United States or Waters of the State. Please contact the US Army Corps of Engineers (USACE) or NC Division of Water Quality (DWQ) if any impacts are proposed to either of these waters.
