This chart may be helpful for you. The 1226 facility had to have it's permit closed and a new one created because the
site address was incorrect. The address it listed was a second part of the property that has been sold. I have not seen
confirmation yet that the new permit has come through.

Thank you again for your help. These are the emails we received a couple weeks ago. They said that someone from the
department would be contacting us to resolve the issues. I see now that one of them is for the facility you called me
about where we were trying to correct the operator name.
