However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on October 11, 2021. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through E, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Residuals Source Certification (FORM: RSC 06-16) Application: 1. No comments. D. Land Application Site Certification (FORM: LASC 06-16) Form: I. The site map for field NC-34-05 shows that there is a reduced setback waiver to the habitable residence on PIN 1514-21-1487 owned by Lois Holmes. However, no setback waiver has been provided for this residence, and footnote 1 of Condition II.8. in the most recent October 13, 2016 permit states that the setback waiver was valid until September 30, 2021. Due to this, the waiver will not be valid during this permit cycle unless re -executed. In order to be able to use the reduced 100-foot setback to this habitable residence under separate ownership, a setback waiver shall be executed in accordance with 15A NCAC 02T A 108(e). This waiver shall be written, notarized, signed by all parties involved, and recorded with the Cumberland County Register of Deeds. A shell of this waiver has been provided with this letter. Alternatively, the site map shall be adjusted to include the 400-foot setback in accordance with 15A NCAC 02T .1108(c), and any reduction in acreage for this fields at this site shall be included in an updated LASC form. E. Landowner Agreement (FORM: LOA 06-16) Form: 1. Section III of the LOA for field CU-34 does not specify whether a re -defined compliance boundary is required for these fields because of a water supply well inside the compliance boundary. Please mark this section to indicate whether a re -defined compliance boundary is needed for this site. If so, the affected fields shall be noted in Section III in accordance with 15A NCAC 02T .0105(h), with the rationale for the requested re -location of the compliance boundary. 2. The LOA for field HA-08-09 did not indicate any need for a re -defined compliance boundary for this site. However, the site map for this field indicates there is a well on this parcel at the western part of this field that could be within the compliance boundary for this field. 15A NCAC 02L .0107(d) prohibits the construction and operation of water supply wells within the compliance boundary. Please indicate whether the well on this field is a water supply well. If so, then Section III of the LOA for this field shall be edited to request a re -defined compliance boundary for this field in accordance with 15A NCAC 02T .0105(h) with the rationale for the requested re -location of the compliance boundary.
