A. Distribution of Class A Residuals (FORM: DCAR 06-16) form:
1. Signature authority delegation:
In the case of a municipal, state, or other public entity, the application form must be signed by
either an executive officer or elected official in the highest level of elected office. [15A NCAC 02T
.0106(b)]. Otherwise, a `Delegation of authority' to sign permit applications to other authorized
employees must be provided in writing to the Division [15A NCAC 02T .0106(c)].
A partially filled `Signature authority delegation letter' has been attached for your reference. Please
have this letter signed by the current City Manager to delegate authority to Mr. Jerry Rothrock,
who is currently in the position of Interim Public Works Director.
2. Program Information:
The residuals to be land applied are classified as Class A 'bulk liquid residuals', and the land application
must adhere to setbacks pursuant to 15A NCAC 02T .1108(b). An information sheet describing the
applicable setback distances in accordance with 15A NCAC 02T .1108(b) must be provided to a land
applier or any person who receives the processed residual [15A NCAC 02T .1109(c)]. Please resubmit the
Information sheet with a description of setback distances.
