I was looking over the additional information you provided on August 9t" 2021. Everything looks good except for one
small issue:
Please correct the following:
- Prior Comment 9 — Since the signing official was changed from Mr. Stempowski to Mrs. Plotsky you will need to
provide a revised, notarized O&M Agreement Form listing Mrs. Plotsky as the responsible party or provide the
official documentation showing that Mr. Stempowski has signatory authority for the Applicant as originally
requested.
Please provide the following:
- Either:
0 1x original singed & notarized hard copy & 1x electronic copy of the revised O&M Agreement Form
listing Mrs. Plotsky as the responsible party, or
0 1x hard copy & 1x electronic copy of some form of official documentation showing that Mr. Stempowski
has signatory authority for the Applicant.

Since these are really minor things, once I get the hard copies, anything else that may have changed from addressing
these issues, and the documents are uploaded to Laserfiche (Provided everything is still in order), we can issue the
permit without doing another formal 30-45 day review.

Chris,
Essentially, we are looking for some sort of paper trail that establishes a link between the person signing the application
and the applicant they are signing on behalf of and ensuring that they have authority to do so. The easiest way to prove
this is to have the person listed on the SoS website sign the form (since it is obvious that they have this authority), but
this is not always practical. If you do not want to have the person listed on the SoS website, you can provide letter
signed by Melissa Plotsky (or a principal executive (vice president or higher)) stating that Brian Stempowski has the
authority to sign this permit (it doesn't need to be an original) or you can provide some form of internal documentation
showing that Mr. Stempowski is a principal executive officer (if he is). If a principal executive officer that is not listed on
the SoS website will provide the letter, you must also provide some form of internal documentation confirming that
individual's position.

Jim,
Thanks for the comments we are working to address all of them. For comment 9. about the signing person, it sounds like
they have to be on the Secretary of State website or provide "official documentation". What does that official
documentation look like? Is it a formal authorization letter or something else? What have you seen in the past? Just
want to know what you specifically need for this.
I think we could get another person to sign the form but that is not preferred because we would have to get all new
original signatures from that person and the land owner.
