Staff of the Mooresville Regional Office (MRO), conducted a compliance inspection for the
referenced permit on May 12th, 2021. The MRO would like to thank you and your AQWA
technicians, Mr. Chad Allen and Mr. David Parker for your time during the site visit. The
treatment and irrigation systems appeared to be operating adequately and the irrigation field
appeared well maintained at the time of inspection.

Inspection Summary:
Staff of the Mooresville Regional Office (MRO), conducted a compliance inspection for the referenced permit on May 12th,

during the site visit. The treatment and irrigation systems appeared to be operating adequately and the irrigation field
appeared well maintained at the time of inspection.

Comment: AQWA cleans and replaces bulbs as needed. This system has a newer UV bulb assembly and
control panel AQWA is now utilizing as repairs are needed. UV bulb amperage is displayed at
the control panel.

Comment: Owner was home and provided a copy of his weekly and monthly checks, but MRO worked w/
his contractors for the treatment and irrigation inspections.

Comment: Application area is grass and appeared well manicured at the time of inspection. Owner is
using two strands of heavy string for ease and safety of removal when accessing field to mow.
The irrigation field appears sufficiently defined and is isolated from surrounding properties.
