to accept your application without this important first step. The Pre -Filing Meeting Request Form is used in accordance
with 40 C.F.R. Section 121.4(a) "At least 30 days prior to submitting a certification request, the project proponent shall
request a pre -filing meeting with the certifying agency" and in accordance with 40 C.F.R. Section 121.5(b)(7), and (c)(5)
all certification requests shall include documentation that a pre -filing meeting request was submitted to the certifying

stone and riprap to stabilize the lake shoreline, which recently has shown

application fee is as follows (pursuant to G.S. 143-215.3D):
$240.00 for impacts to lake (below normal water level) of less than 1 acre
$570.00 for impacts to lake (below normal water level) of greater or equal to 1 acre

The shoreline off the Lake Roxboro at 135 Lake View Road, Prospect
Hill, NC in Caswell County has shown some erosion over the last a
few years. The City of Roxboro owns the property's shoreline. As the
owner and also an environmental engineer, my concern is that this
will continue due to increased boat travel. I would like to hire a
contractor to perform the shoreline stabilization, including grading,
installing base stone, geotextile mat, and Rip Rap. The total length is
approximately 200 feet.
Based on the direction of the Lake Warden, Mr. John Williford of the
City of Roxboro, I would like to check with the regional
office with regards to pertinent process and permit application form,
etc., to proceed the project.
