Maureen was unable to find anything showing the facility was Class II except for some old designation forms (see her response below). Would ARO happen to have any letters or knowledge on why the permit and BIMS appear to be in conflict? I dug through the physical file from the file room in Archdale and found a 1991 WLA which states the facility is Class I per the facility classification determination worksheet attached to it. However, I also found an old Staff Review and Evaluation sheet attached to the 1996 permit issuance which states the facility is Class II and proposes the change in monitoring frequency for BOD, TSS and Fecal from 2/Month to Weekly (which follows 15A NCAC 02B .0508). There's also an email from Wanda Frazier in 2010 stating "Classification: Class II facility (not: Class I — as indicated in your email below)". It's my guess that the facility was originally classified as Class I, but somehow was either re-classified to Class II (which we can't find a letter to confirm this) or the permit writer documented it as Class II in 1996, either purposefully or in error, and the monitoring frequency hasn't changed since then. I'm hoping ARO might have some other information that can help shed some light on this. It appears to be a WW-I; per permit. Perhaps the regional office has some history on this?? Central Files permit file? I've been here since 2015 so if it's not in BIMS or the designation files, I can't help. I will say a look at the designation forms that we have from 1995-2004 note WW-II. I don't know if that is an error on their part or not. They would have (or should have) received a letter of classification and then a letter of reclassification. If they have neither...maybe the regional office is a good source for the history (Linda Wiggs). Our agency maintenance person has been out with Covid and is now in the ICU...so I need to make sure all my bases are covered. Environmental Inc. said that they get the forms from you guys. If they can still do that for the monitoring, the rest can come to me. I did contact the Asheville office about a month ago because the information on the form was incorrect. They have the Evergreen address as 100 Silver Bluff, Canton NC 28716 and the phone listed as 828-648-2044. The rest of the information (facility name and number) is correct. The mailing address for the Evergreen Foundation is 28A Oak St., Waynesville, NC 28786 and the office phone is 828-456-8005 This past year we didn't get the renewal and when I heard through Environment Inc that it was past due, we had just a couple of days to get it in.
