The purpose of the site visit was to check the condition of the plant and infiltration fields since the last inspection on May 18, 2021. Robert Howard, operator of Fortin Contract Services, was present during the visit. A Notice of Violation and Intent to Assess Civil Penalty was issued for the routine inspection conducted on May 18, 2021. The following violations were noted: 1. Lack of maintenance of rotary distributer systems. 2. Measures not taken to prevent effluent ponding in the infiltration sites. 3. Lack of maintenance on the EQ tank to ensure proper operation. 4. Monitoring wells not secured. 5. MW-1A has not been installed to replace MW-1 as required in the current permit schedule. MW-3 and MW-1 have not been permanently abandoned as required in the current permit schedule. It appears, from the recon site visit, that the above violations have not been addressed. Because of the lack of maintenance of the EQ, effluent from the clarifier on the east side was bypassing the tertiary filter unit by a portable pump to the wet well before the chlorine contact tank. In addition, the plant is in an advanced state of corrosion in many areas causing a safety hazard. Structural integrity needs to be evaluated. Please refer to the enclosed photos. The facility is not compliant with Non -Discharge Permit WQ0004059. Please respond in writing to this office within 10 days of your receipt of this letter regarding your plans or measures to be taken to address the issues noted above.
