I was looking over the additional information you provided on August 17t" 2021. Everything looks good except for a few
small issues:
Please correct the following:
- Prior Comment 5 — Please provide original signed hard copies of both the Supplement-EZ Form and the O&M
Agreement Form. Photocopies of both of these forms were provided in the calculation booklet, but the original,
signed hard copies were not provided. Please provide them.
- Prior Comment 15j — Plansheet C8.17 indicates that the permanent pool of this wet pond is at elevation
1,210.50 ft and the bottom of the sediment storage at the forebay exit is shown at elevation 1,205.75 ft. This
would imply that the depth of the forebay at the exit (Line 44 on the Wet Pond Page of the Supplement-EZ Form
is 57 in. (54 in. is shown on the form).
- Prior Comment 16 — Please make sure that the electronic files from the prior (8/17/2021) submission are
uploaded through the provided link. We did not receive any electronic files from the previous submission (only
the hard copy submission). The electronic files are required per 15A NCAC 02H .1042(2).
Please provide the following:
- 1x electronic copy of all of the submittal items from the 8/17/2021 submission (planset, calculation booklet,
Application, response to comments letter).
- 1x original signed hard copies of both the Supplement-EZ Form and O&M Agreement Form.
- 1x hard copy and 1x electronic copy of the revised wet pond page of the Supplement-EZ Form.

Since these are really minor things, once I get the hard copies, anything else that may have changed from addressing
these issues, and the documents are uploaded to Laserfiche (Provided everything is still in order), we can issue the
permit without doing another formal 30-45 day review.

for the subject project on July 19t", 2021. A review of that information has determined that the application is not
