Box 33068 Raleigh, North Carolina 27636 -3068 Following the field verification meeting on April 24, 2012, on behalf of our client, the N.C. Department of Transportation (NCDOT), Kimley -Horn and Associates, Inc. (KHA) is submitting a revised Preliminary Jurisdictional Determination (JD) package for two of the bridge replacement projects (B -5360 and B -5546) located in Randolph County. No revisions were required for B -4608 and a Preliminary JD was issued for this project on April 25, 2012 (SAW -2011- 2348). The following is a summary of the discussions for each of the project sites conducted during the field verification meeting: TIP# B -4608: Proposed replacement of bridge number 208 on SR 1003 (Erect Road) over Fork Creek. The USACE/NCDWQ concurred with the four jurisdictional streams (referenced on Figure 3 as Fork Creek (SA), SB, Reedy Creek (SC) and SD) and one non - jurisdictional ephemeral stream (SE). It was agreed that mitigation ratios for impacts to perennial streams Fork Creek (SA), Reedy Creek (SC), and SD would be 2:1. SB was determined to be an intermittent stream with a 1:1 mitigation ratio requirement. TIP# B -5360: Proposed replacement of bridge number 374 on SR 2481 (Low Bridge Road) over Sandy Creek. The USACE/NCDWQ concurred with the two jurisdictional streams (referenced on Figure 3 as Sandy Creek and SC). The feature that was originally delineated as an ephemeral /overflow channel (SB) was determined by the USACE to be a wetland floodplain pool (WA). The revised Preliminary JD package reflects this change and includes a USACE Wetland Determination Form and NCDWQ Wetland Rating Form for the delineated feature. It was agreed that mitigation for impacts to Sandy Creek would have a 2:1 ratio and impacts to intermittent stream SC would be 1:1. Impacts to wetland (WA) would have a 1:1 mitigation requirement. TIP# B -5546: Proposed replacement of bridge number 307 on SR 2481 (Low Bridge Road) over Mt. Pleasant Creek. The USACE/NCDWQ concurred with the two jurisdictional, perennial streams identified in the study area (referenced on Figure 3 as Mount Pleasant Creek (SA) and SB (downstream/south of Low Bridge Road). It was agreed that mitigation ratios for impacts to these perennial features would be 2:1. It was determined that the jurisdictional extent of the intermittent reach of SB (upstream/north of Low Bridge Road) should be extended to the west where the channel enters the project study area via a culvert underneath a residential driveway. The revised Preliminary JD package reflects this change. Due to the disturbed nature of the upstream, intermittent reach of SB, it was agreed that mitigation would not be required for impacts to this channel.
