The Division of Water Quality has reviewed your submittal fora 401 Water Quality Certification for the
aforementioned project. A brief review of your application revealed it lacking necessary information. The permit
application was deficient in the following areas: ,,
• The application was not signed by an authorized representative
Therefore, pursuant to 15A NCAC 2H .0502(f), we are returning yow permit application. The NCDOT has the
option to re-submit a signed application for this project. If a signed application is received by the DWQ, then the
application will be reviewed further prior to issuance of a 401 Water Quality Certification.
