On December 14, 2006, Mr. Daryl Lamb of DWQ's Winston-Salem Regional Office visited this site in order to assess the proposed impacts. Based on the information provided in your application and Mr. Lamb's site assessment, additional information is required in order that we may determine your project's compliance with 15A NCAC 2H .0500. Please provide five (5) copies of the following information: 1. The proposed impact to 247 linear feet of intermittent stream consists of the placement of 24" reinforced concrete pipe in the stream. This stream is located within the Randleman Lake Water Supply Watershed and is subject to the buffer requirements described in 15A NCAC 02B .0250 Randleman Lake Water Supply Watershed: Protection and Maintenance of Riparian Areas. Please provide information which shows that the proposed impact will not violate the requirements of 15A NCAC 0.2B .0250. 2. With the consent of the U. S. Army Corps of Engineers (USAGE), you are proposing to provide onsite compensatory mitigation by restoring 416 linear feet of onsite degraded stream channel which includes the areas of two small drained ponds. You are requesting authorization from the USAGE to use Nationwide Permit 27 for the impacts associated with this restoration. The 416 linear feet of channel are not listed in the table for stream impacts on Page 8 of the Pre- Construction Notification (PCN). Please provide a corrected table of impacts that lists all proposed stream impacts on the project site and also provide a corrected table of cumulative impacts, which is on Page 9 of the PCN. Also, this project will be placed on hold for DWQ processing due to incomplete information (15A NCAC 2H .0507(h)). The processing of this application will begin when the requested information is received. Please be advised that if your proposed activity is subject to Section 404 of the Clean Water Act (33 U.S.C. 1344), you must also receive a Section 401 Water Quality Certification from the N. C.
