However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on September 24, 2021. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through C, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Please reference the subject application number when providing the requested information. All revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper copies and one electronic copy submitted to my attention at the address below. The site maps for Field NC-PI-01, Fields 1-2,6 depict two onsite houses, one to the east of Field 1 and north of Field 2B, and the other to the south of Field 2A. There are concerns that there are water supply wells at these houses that were not depicted on the site map. There has already been confirmation that there is a well at the northernmost house, which shall be included in the map with the 100-foot setback as required by 15A NCAC 02T .1108(c). The house to the south shall also be checked to verify if there is a water supply well serving this house. If there is a well, the well's location shall be indicated on an updated site map with the accompanying 100-foot setback. B. Land Application Site Certification Form (FORM: LASC 06-16): 1. See comment A.1. If the 100-foot setbacks for these wells infringe on the land application fields, a reduced acreage may need to be provided for any affected field. C. Land Owner Agreement Form (FORM: LOA 06-16): 1. If there is a water supply well to the south of Field 2A, it would likely be located within the compliance boundary of this field as defined in 15A NCAC 02L .0107(b). In accordance with 02L .0107(d), no water supply wells shall be operated within the compliance boundary of a disposal system. However, in accordance with Item III of the LOA 06-16 form and 15A NCAC 02T .0105(h), these compliance boundaries can be placed closer to the waste disposal area, such that the water supply well is situated outside of the compliance boundary. If there is a well serving the onsite house south of Field 2A that is located within the field's compliance boundary, Section III of the LOA shall be modified to include the request for a re -defined compliance boundary for this field.
