Dear Mr. McMillan:
This submittal contains revised exhibits and additional information needed to answer the deficiencies
noted in a letter from Steve Tedder, Water Quality Supervisor of May 26, 2006. The original permit
application for this work was submitted to the USACE for Nationwide permits 14 and 27 and was
approved on June 16, 2006, signed by Todd Tugwell of the USACE and assigned ID numbers 200221316
and 200620838. Mr. Tedder's letter noted deficiencies that would need to be addressed before the 401
Water Quality Certification could be approved. We communicated with Sue Homewood in the Winston-
Salem office to discuss and clarify the items mentioned in Mr. Tedder's letter.
Deficiencies for Water Quality Certification were noted as items from 1 to 8 in Tedder's letter. Those
deficiency items required some additional or revised information and subsequently several exhibits have
been prepared to clearly answer the deficiencies. The items below address the deficiencies by
corresponding to the same numbering system as Tedder's letter:
Item 1. The impacts to the streams have been broken down into two (2) descriptions, remaining listed
as separate streams. A table is used to identify stream lengths as relocated stream and enhanced
stream lengths. Total impacts are noted separately for the South and North Tributaries for clarity.
The South Tributary has been noted "Main" on the exhibits as requested. See Exhibit #6 -Proposed
Stream Relocation/Restoration Plan (Revised) and Exhibit 11 (new)
Item 2. Grade control sills are utilized at the upper end of both channels to provide channel stability
where the existing and revised stream channels join. Greg Jennings recommends the use of grade sills
in this location because of the dams upstream. He recommends grade control in the event when extra
flows occur at this location. Grade control sill details have been added in Exhibit #10 -Proposed
Stream Relocation/Restoration Details (new sheet)
Item 3. A copy of the Plan and Profile sheet (G-6) reduced from the original (30 scale) to an 11 x 17
(approximate 60 scale) is included with this submittal. Exhibit #8 was prepared to provide schematic
channel sections for clarity and Exhibit #6 has been revised for correct scale and stream alignment.
Item 4. Exhibit #9 -Proposed Stream Relocation/Restoration Thalweg Details (new) has been
prepared and is included in this submittal
Item 5. Exhibit #8 -Proposed Stream Relocation/Restoration Typical Sections (new) have been
prepared to better represent the proposed North and South stream relocation and restoration sections.

Slopes will be 2:1 at the replacement culvert and blend to 3:1 slopes as soon as possible at the road
bank to the toe of slope. See also the 11x17 reduced copy of Plan and Profile sheet G-6.
Item 6. Deleted Exhibit 7 and replaced with Exhibit 10 which includes cross vane structure details
with notes for bankfull and stream bed elevations, references to Plan and Profile sheet G-6, Root Wad
and Grade Control details.
Item 7. The North Tributary stream alignment including the meander bend of concern has been
revised away from the toe of slope to add a minimum 10-ft distance between the top of streambank
and the proposed toe of slope. This will allow for vehicular traffic and provide adequate distance
between the rootwad area of the streambank and the proposed road bed fill for increased stability. See
Exhibit 6 and the 11x17 reduced copy of the Plan and Profile sheet G-6.
Item 8. The Conservation- easement has been revised slightly to provide the 100-ft buffer as is
feasible. The easement boundary is shown on Exhibits 6 and 11. Exhibit 11 indicates planting
guidelines for undisturbed and required buffer planting areas. An 11 x 17 inch reduced scale
(approximately 60 scale) copy of the Landscape plan PL-1 provides identification for buffer and
stream vegetative planting material. The existing sanitary easement is also shown on the plans and
exhibits and will be utilized for construction access during a portion of the stream relocation work.
We believe this revised and additional information addresses all the deficiency comments with regard to
the 401 Water Quality Certification. If you have any questions or need additional information, please feel
