However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on September 23, 2021. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through D, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Please reference the subject application number when providing the requested information. All revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper copies and one electronic copy submitted to my attention at the address below. Section I — Applicant Information: The Applicant listed in this section is Waters Edge Property Homeowners Association on High Rock Lake ("Homeowners Association"), which is different than the previous Permittee, Water's Edge Property Owners Sanitary Sewer Association ("Sanitary Sewer Association"). The Secretary of State website indicates that Sanitary Sewer Association has changed their legal name to Waters Edge Property Owners Association on High Rock Lake ("Property Owners Association"), which isn't exactly what is listed in the application. Please clarify whether the Homeowners Association is the same entity as the Property Owners Association, and whether Mr. Cameron Spencer is considered the President of this entity. B. Section VIII — Monitoring Wells: The Division is concerned that monitoring wells MW-1 and MW-3 have become backfilled with sediment which may adversely impact the data reported at these wells. The Division requests that well integrity testing is performed on both monitoring wells MW-1 and MW-3 to determine if these wells are open to the aquifer or have become plugged, collapsed, or otherwise not functioning properly. These tests shall be performed by a North Carolina certified well contractor. The results of these tests shall be submitted for the Division for review. C. Attachment E — Operational Agreement: 1. The Home Owners' Association Operational Agreement included in the application was dated the 15t` of September, 2015. This Operational Agreement shall be renewed with this permit renewal and shall be re -executed. 2. Pursuant to 15A NCAC 02T. 0115(c), legally formed Homeowners' or Property Owner's Association shall submit a copy of the Articles of Incorporation, Declarations, and By-laws with permit application. This information shall be included with the executed Operational Agreement in Item C.1. D. Attachment I — Compliance Schedules: Compliance Schedule I.2. of the previous permit required that a revised facility map showing the clearly demarcated spray irrigation field and area of these fields shall be prepared and submitted, and if the area of the spray irrigation field requires a change from the value shown in Attachment B, a request for a modification shall be filed with the Division. The revised map appears to show the spray coverage area listed as 4.9 acres, which is different than the 7.0 acres listed in the previous permit's Attachment B and the total of 6 acres listed in the application under Section IX — Irrigation Fields. Additionally, this map doesn't appear to have a distinction between Fields 1 and 2. Please clarify the acreages of these fields, and provide a distinction of the two different fields on the map with each field's acreage.
