Provide a brief description of other controls that manage/prevent/minimize Stormwater runoff (e.g., erosion and sediment controls, inlet protection/control at storm drains, diversion structures): Hartley Ready Mix Concrete uses a two truck mixer pits that contains all process water. Hartley Ready Mix Concrete also uses a settling basin to capture all debris rom entering the stream. Part 2 —Stormwater Discharge Outfalls (SDOs) 1. Any evidence of discharge to receiving waters/MS4 from the SDOs? (e.g.: stonnwater runoff, dry weather discharge, co -mingling of process wastewater, trash, debris, oil and grease): ❑Yes ®No On August 11, 2021, Stormwater staff noticed dry weather discharge from the settling basin that had particles which ultimately discharged into Salem Creek. Hartley Ready Mix discharges directly into Salem Creek instead of entering the M84 system. 2) Do the Stormwater outfalls on -site correspond with those listed on the site map in the Stormwater Pollution Prevention Plan (SWPPP)? ®Yes ❑No If no discharge occurs during the sampling period, the permittee must record "No Flow" or "No Discharge" within 30 days of the end of the sampling period in the facility's monitoring records. Please clean up and properly dispose of the sediment on the ground outside of the wellhouse to prevent runoff into the stormwater drainage system. 2. Please install a dumpster plug at the bottom of the dumpster downhill from the we Ouse to prevent the leakage of water mixed with trash from inside the dumpster. 3. Please properly dispose of the barrel holding dirty water located behind the Douglass Battery Trailer adjacent to the train tracks. 4. Please clean up and properly dispose of the trash and debris located on the graveled corner of the property to prevent possible transport by rain events into the stormwater drainage system. 5. Please properly dispose of the broken white barrel bottom holding dirty water located next to the stormwater water sampling site on the property near the fence line. 6. Please dispose of the orange bucket sitting in front of the parked forklift, and please clean up oil based substance spilled on the ground around the bucket and forklift. Please determine a way to prevent the forklift from leaking. 7. Please clean up and properly dispose of spare parts on the ground and on the pallet and trash that is under the conveyor belt. 8. Please clean up and properly dispose of gravel that is underneath the concrete truck that is awaiting to be repaired or disposed of which is beside the settling basin near the back corner of the office. 9. Please clean the gravel, sediment, trash, and debris from the entire stormwater channel that is a part of the settling basin. Also, please remove all trap and contained dirt at the discharge point of the settling basin to prevent stormwater pollution. 10. Please clean up all fresh all oil spots on the with oil dry and dispose of all the sorbents and any other materials used to perform this action. 11. Update and sign the responsible party with signature and dates. 12. Please provide some employee training for the staff who will be involved with the SPPP. 13. Start performing analytical and qualitative monitoring for each SDO. Also, document the records in the SPPP. 14. In the SPPP, please list the major spills that have occw•red within the past three years. If no spills have occurred, please document that in the SPPP. 15. Please update and sign the Responsible Party section of the SPPP. 16. Provide a signed, annual certification that the facility has been tested for non-stormwater discharges from the site of Hartley Ready Mix Concrete Mfg. 17. Implement the preventative maintenance and good housekeeping program. 18. Please provide inspection records of cleaning and preventative maintenance. 19. Update the spill prevention and response procedures with the identified, responsible personnel with signatures and dates. 20. Please update SPPY to current year and date. 21. Please increase the good housekeeping cleaning frequency to reduce Stormwater pollution. Also, please provide the newly increased frequency in the SWPPP. 22. Please submit the no Flows and no discharges to NC DEMLR from the past permit year and provide the justifications. 23. Please conduct a facility inspection for the second half of the year which is July to December. 24. Please update the Stormwater Pollution Prevention Plan with the current team members in place that will be overseeing it since the plan has not been updated since 2015. 25.
