We find the plan to be acceptable with modifications and hereby issue this letter of Approval with Modifications. The Modifications Required for Approval are listed on the attached page. This plan approval shall expire three (3) years following the date of approval, if no land- disturbing activity has been undertaken, as is required by Title 15A NCAC 4B .0129. Please be advised that Title 15A NCAC 4B .01 1 S(a) requires that a copy of the approved erosion control plan be on file at the job site. Also, you should consider this letter to give the Notice required by G.S. 113A- 61.1(a) of our right of periodic inspection to insure compliance with the approved plan. North Carolina's Sedimentation Pollution Control Program is performance- oriented, requiring protection of existing natural resources and adjoining properties. If, following the commencement of this project, it is determined that the erosion and sedimentation control plan is inadequate to meet the requirements of the Sedimentation Pollution Control Act of 1973 (North Carolina General Statute 113A -S 1 through 66), this office may require revisions to the plan and implementation of the revisions to insure compliance with the Act. In addition, local city or county ordinances or rules may also apply to this land - disturbing activity. This approval does not supersede any other permit or approval. Silt fence shall be installed parallel along the stream channel leading to the proposed 48" RCP during the clearing and grubbing phase. The silt fence must remain intact until road fill placement begins. 2. The outlet protection below the 48" RCP shall conform to the existing channel. 3. A rock check dam with a T top crest width and maximum T height shall be installed at the limits of disturbance below riser basin #5. This measure must remain intact until the outlet protection for the barrel outlet is installed. 4. Inlet protection shall be provided for the 18" RCP off Old Way Rd. S. In the Construction Sequence for installing the 48" RCP, the coffer dams shall be installed prior to any disturbance and must remain intact and functional until all culvert construction is completed and stabilized. This includes installing the outlet protection below the culvert outlet. 6. Rip -Rap Donut I=nlet Protection shall have a minimum top crest width of 3' and constructed of Class B stone. 7. Riser Basin #1 shall have a 5" skimmer with a 4" orifice, Basin #2 shall have a 2.5" skimmer with a 2.2" orifice, Basin 43 shall have a 2.5" skimmer with a 2.4" orifice, Riser Basin #4 shall have a 4" skimmer with a 3.3" orifice, Riser Basin #5 shall have a 4" skimmer with a 3.0" orifice. 8. A' I'll need the calculations and dimensions of anti- floatation for riser basins on detail. V!� Need to provide elevations for riser basins ( #1, #4, #5) of the following: top of dam, top of riser, bottom of basin, emergency spillway. You can place this in a Table on Sheet 3 above your detail. I'll need hard copies of all the plans by Thursday evening. Provide topo contours for the excavation of Riser Basins #1 & ##4 and Skimmer Basins #2 & #3, Ensure the proposed fill does not encroach into the measures. 2. Clearly show riser/barrel systems an plans with skimmer attached. In addition, clearly show skimmers attached to barrel pipe for skimmer basins. 3. Provide a scale on the plans that is legible. An overall sheet (to show entire site disturbance) at 1 " =150' is sufficient, however, a larger scale of 1 " =30' or 1 " =40` is needed to clearly see proposed contours. Be sure to provide match Iines if areas extend to other sheets. 4. The clearing limits cannot be shown east of the roadway across the stream as shown. Currently, this shows clearing both hillsides and stream channel below the 48" RCP. Revise plans as needed. S. Provide positive drainage in proposed diversion berms to skimmers/basins (e.g, skimmer basins #2 & #3, Basin 44, possibly Basin #1). 6. Provide an adequate measure in drainage channel northwest of skimmer basin 92. Recommend placing riser basin #1 in this location and providing a smaller basin where riser basin #1 is currently being shown. 7. Adequate measures are needed for the initial clearing & grubbing for roadway across stream. In addition, adequate measures are needed for the grading of proposed roadway. The silt fence in the slope cannot be maintained as shown. 8. Provide a cofferdam and pump around detail for placing 48" RCP in anon-flowing stream. In addition, provide a construction sequence for installation of culvert in the dry. 9. Provide a delineation map to show the drainage areas to proposed measures in the existing conditions and graded conditions. The proposed 24" RCP northwest of skimmer basin #2 must be extended to outlet at the toe of proposed fill slope. 11. The ford stream crossing is not allowed for equipment (delete details). Provide an adequate equipment crossing and/or show proposed culvert to be installed and utilized. 12. Provide anti - flotation calculations for riser basins.
