Good 7: Fair Corrective Action is needed to resolve the situation to full 6 -0: Immediate Corrective Action !Needed compliance with the Law: (TISA: 048.0000) Permit Consultation Comments: A "PCN" is being issued to document off -site sedimentation into jurisdictional areas. Mason Herndon with DWQ has been contacted and has advised contractor to remove sediment from streams by hand. Remarks and Recommendations: Rode through project with Barry Brewer, wIDOT and Jerry wlBarnhill. Project was reviewed on May 17th & 19th. Project is out of compliance due to failure to maintain erosion control devices, failure to take all reasonable measures to prevent off -site sediment, and failure to follow approved plan. Please note the following corrective action at the three off -site locations. 1) US 220(Y12) - approx. sta.65 +00 Rt. on Y12 - off -site sediment has occurred in this location. Contractor will need to remove sediment from outlet ditchlstream that exits into jurisdictional stream. Also, need to remove sediment in two locations in major stream. I talked to Mason Herndon wIDWQ on May 20th and he gave us permission to remove sediment in this area by hand. Need to install special sediment control fence in low area at silt fence failure, asap and seed/mat slope. Recommend installing additional slope drain and basin, as shown on plans at 01, as discussed to divide up drainage area on subgrade. Also, install TRSC at end of rock -lined ditch. 2) US 220(Y12) - (This site was reviewed on May 17th and again on May 19th.) Approx. sta.79 +50 Rt. on Y12 - contractor has started hauling into this section. During storm event over the weekend, sediment was lost off -site into stream due to silt fence failure. Mason Herndon w /DWQ was notified on Monday and indicated sediment deposits could be removed form stream. Sediment deposits were removed from stream on May 18th. There continues to be one area that needs sediment removal at stream and also, need to remove sediment control stone and small amount of sediment at right -of -way fence. At time of review, today, contractor has installed additional slope drains and basins, as discussed on May 17th. Need to seedlmulch slope, asap. Approx. sta.79 +50 Lt. on Y12 - contractor has performed maintenance to silt fence and basins. Seed /mulch disturbed areas, esp. slope, asap, 3)"Sta.450 +00 Rt. at culvert site - need to remove sediment in stream at old temp. stream diversion, re- construct stream bank, and seed/mat. Need to remove silt fence at outlet end of top drainage pipe and install scour hole, as shown on plans. Recommend diverting runoff into woods until streambank is stabilized. Also, need to remove blockage inside culvert. 4) Please see additional corrective actions that need to be addressed throughout project. Approx. sta.135 +00(Spencer Rd.) to Banner Whitehead Rd.(Y3) - contractor continues to actively grade in this section. Approx. sts.133 +00 to sta.141 +00 Rt.- contractor has seeded /mulched ditchline. Repair washes and seed/mat. As work continues, stage seed /mulch slopes. Ensure runoff is diverted into skimmer basin(ID.- 7.3CG) and drainage areas are divided up, as not to overload one basin. "Sta.147 +50 Rt.- contractor has installed rock plating on slope at inlet end of pipe. Need to remove rock from bottom of stream channel and stabilize slope Rt. of finished ditch at wingwall wlseed /mat. Roadside Environmental Unit Immediate Corrective Action Ensure inlet protection is installed as boxes are constructed and install silt basins, as shown on plans. Sta.149 +00 to sta.153 +50 Rt.- berm on back side of skimmer basin(ID.- 8.1CG) has been repaired. Contractor has NOT installed wrapped TRSC's and wattles wIPAM to help with turbidity, as discussed. *Sta.164 +00 at culvert site - contractor has not hauled into low area since Nov. Contractor has completed dry detention /HSB between -L- and -SRI- to help with storage in this area. Skimmer basin(ID.- 9.1CG) - need to install wrapped TRSC's and wattles wIPAM to help with turbidity issues. Recommend installing these devices with PAM at all outlet basins in this section due to turbidity concerns at culvert dowmgrade. *Sta.175 +00 Lt. at outlet - need to clean out small amount of sediment at outlet of pipe and perform minor maint. to special sediment control fence. *Sta.179 +00 Rt. at inlet end of culvert - need to ensure slope at culvert is graded, properly and perform maint. to special sediment control fence. Once complete, seed/mulch. Ensure stope drains and berms are installed, as needed to divert runoff to basins. * Approx, sta.197 +00 at crossfine pipe - approx. sta,196 +00 Lt.- contractor has installed TRSD at end of TSD to protect pond downstream. Seed /mat small area around headwall area at inlet end of pipe. Ensure PAM is installed on wattles after each .5 rainfall event. Temp. seed/mulch, as needed to meet 21 day seeding requirement. Banner Whitehead Rd(Y3) to Nelson Park Rd.- contractor temp. mulched disturbed areas a couple of months, ago. Skimmer basin (ID.- 59.6F) - berm on side of basin has been built up to prevent over topping. *Sta.238 +00 at culvert - Rt. of -L- continue to maintain temp. diversions in this area. Recommend additional wattles wIPAM upgrade of skimmer basin Lt. of -L-. Continue to temp. seed/mulch disturbed areas. Nelson Park Rd. to Branson Davis Rd.(Y5) ? contractor continues to haul and actively grade in this section. *Sta.269 +00 Lt.- off -site sediment occurred in this location on May 6th due to undermining of box and pipe of dry detention basin. Sediment has been removed, as per Mason Herndon ?s recommendations. Recommend seeding disturbed areas in this location with our native seed mix wlno straw. Need to install special sediment control up and around headwall area and seed/mat slope, asap. Also, need to seed/mat slopes inside dry detention basin. Recommend adding wattles wIPAM at slope drains at entrances to dry detention basin to help wlturbidity. As work continues stage seed/mulch slopes and temp, seedlmuich, as needed. *Sta.280 +00 Rt. at permit site 18 - contractor has drained pond and has brought up fill. Pond is starting to naturally green up. Disturbed areas at inlet end of pipe may need to be seeded with native seed mix. Stage seed/mulch disturbed slopes Rt. of -L -. Branson Davis Rd. to existing US 311(Y7) - sta.300 +00 to sta.309 +00 - disturbed areas needs to be temp. seeded/mulched. Ensure temp. diversions are installed to divide up drainage areas. Walker Mill Rd.(Y6)- contractor has turned traffic onto detour. Need to perform maint. to TRSC's and repair ditch erosion and seed/mat. *Sta.321 +00 Lt.- plan is to ensure weather conditions are good before hauling fill into this area to construct roadbed and filling in existing skimmer basin(ID,- 21.1). Once base of fill is in, contractor will need to install a long tiered skimmer basin with required storage to provide protection until slope and roadbed are stabilized. Ensure weakly vegetated fill slopes and median sections are supplmantallrepair seeded/mulched. Perform general maint. to type C inlet protection throughout this section. Interchange area - RP2C at *sta.23 +00 Rt.- stream bank has been rock - plated and is holding up. Dry detentionlHSB #13 - repair washestdisturbed areas on banks and seed/mat. RP2B at sta.22 +75 Rt.- additional basin has been installed. Buffer area looks good at this time. Also, need to re- install orange fencing in this area. Continue to temp. seed/mulch disturbed areas throughout interchange area. US 311(Y7) - contractor has turned traffic onto new bridge and is constructing RR bridge at this time. Repair ditch erosion and seed/mat ditchline on US 311 at RP2B. Existing US 311 to Plainfield Rd(Y8) - *Sta.385 +40 Lt.- Mason Hemdon looked at site on May 110th and indicated additional sediment deposits need to be removed. Need to temp. seed/mulch disturbed median section. Install silt basins at DI's, as shown on plans. SR3 - contractor has seeded/mulched shoulders and installed silt devices inside DI's. Need to continue to install inlet protection, as shown on plans, also. Disturbed area - approx. 60+ acres. *HQW area* Plainfield Rd.(Y8) to Old Courthouse Rd. - contractor has completed bridge and has started grading on Y8. Continue to install e. c. measures and seed/mulch disturbed areas. SR4 - contractor is working in this area. Continue to seed/mulch disturbed areas. Old Courthouse Rd. to Heath Dairy Rd.(Y10) - #install silt basins at DI's throughout this section, as shown on plans and seed/mulch disturbed median section, asap. *Sta.450 +00 Rt. at culvert site - perform corrective actions, as stated above. Heath Dairy Rd.(Y10) to US 220 - contractor has completed bridge, graded, and seeded/mulched on Y10. Repair seed/mulch slopes Rt. of Y10. Need to repair seed/mulch weakly vegetated slopes or disturbed slopes throughout this section. * Approx. sta.470 +00 Lt. at permit site 28 - discussed with DWQ & COE last year about diverting stream in old channel to align with pipe. This work can proceed once areas are on subgrade are stabilized. Need to repair washes in ditchfines and slopes and seed/mat throughout this section. US 220(Y12) interchange - RP3C - continue to perform maint. to wattles at slope drains. RP313 - repair washes on slopes and seed/mat. Repair washes on slopes throughout interchange and seed/mat. Approx. sta.79 +50 Lt. on Y12 - contractor has performed maintenance to silt fence and basins. Seed /mulch disturbed areas, esp. slope, asap. RP3CA at *sta.36 +50 - rock cross vane was installed last fall. Continue to monitor for stabilization. Disturbed area - approx. 15 acres. General comments: Contractor continues to actively grade, esp. on the north end of project and on south and at US 220. As grading work continues, ensure disturbed areas are reviewed at the end of each workday and install temp, diversions or additional e. c. measures, as needed to ensure runoff is managed and diverted to skimmer basinsrrRSD's. Drainage areas should be divided up, as not to allow runoff to overload one single measure in sensitive areas and result in off -site issues. A. Roadside Environmental Unit Immediate Corrective Action Contractor needs to ensure erosion control measures are installed and maintained, as per erosion control plan and NPDES requirements. Seeding contractor is performing repair seeding work on project at this time. Continue to repair washes and ditch erosion throughout project and seed /mat these areas, as well as weakly vegetated slopes. This work needs to be completed in the next few weeks to get good germination of seed before hot weather conditions. Continue to stage seed/mulch slopes as work continues and temp. seed/mulch, as needed to meet 21 day seeding requirement. Also, ensure safety fence is maintained and re- installed, as needed throughout project. NPDES documentation was not reviewed due to lack of time. Continue to complete NPDES documentation weekly and within 24 hrs. after an .5 or greater rainfall event. Also, continue to update clearing/grubbing and final e. c. plans. Good 8. Good 7: Fair 6 -0: Immediate Corrective Action !Needed The " ICA" status on this project has been lifted at this time. A follow -up review was conducted on May 27, 2011 with Jeff Loflin, Resident Engineer, John Partin, Asst. Resident Engineer, Vickie Davis, Roadway Construction Engineer, Tim Latham w /Land Quality, DOT inspectors, and contractor personnel that indicated corrective actions have been taken to bring the project back into compliance. Majority of issues on the project have been completed at this time. Please note the following comments and recommendations: 1) US 220 at approx. *sta.65 +00 Rt. on Y12 - sediment has been removed from stream, as per Mason Herndon w /DWQ. Need to continue to monitor ditch /stream and remove additional deposits as they migrate down after each rain event. Erosion control measures have been installed, as per plans. 2) US 220 at approx. *sta.79 +50 Rt on Y12 - off -site sediment has been removed, except for one area that needed additional attention. Erosion control measures have been installed and maintained and slope seeded /matted. 3) *Sta.450 +00 Rt, at culvert site - sediment has been removed from stream and bank seededlmatted. Recommend diverting runoff from down pipe into woods to avoid future erosion on stream bank. Also, need to final grade areas up and around culvert headwall area, re- install silt fence /special sediment control fence, and seed/mat. 4) *Sta.147 +50 Rt.- need to re -grade and stabilize small slope area Rt. of finished ditch at wingwall. Also, need to re-seed/mat ditch bank adjacent to right of way line. *Sta.269+00 Lt.- need to seed disturbed areas with native grass seed mix. Also, need to re- install orange fencing in permitted area. From Branson Davis Rd. to existing US 311(Y7) - continue to repair seed /mulch weakly vegetated areas throughout this section. Dry detention basin #13 - repair washes /disturbed areas on slopes and seed /mat. US 311 - repair ditch erosion in ditchline on US 311 at RP213. General comments: As hauling and grading continues on project, please ensure at the end of each day, that lifts of material in fill sections are coordinated to ensure runoff on subgrade is directed into skimmer basins. The contractor may have to install temp. diversionslberms or additional slope drains depending on subgrade, as needed to divert runoff to skimmer basins in a controlled manner. Ensure erosion control measures are installed, as shown on plans, esp. silt basins at DI's and e.c. measures are maintained, as per NPDES regulations. Continue to stage seed/mulch slopes and median sections, as soon as possible and temp. seed/mulch, as needed to meet 21 day seeding requirement. 5131111 3:11 Page i of 1
