This memo is a follow -up to our site visit of the 311 Bypass project in Randolph County on May 10, 2011.
I appreciate the time you and other NCDOT staff, as well as Barnhill Construction spent with me to review
this project.
The following comments are provided to help document our joint site visit and to provide technical
guidance and assistance to DOT staff. Only sites 18 and 13 were reviewed jointly, all other sites that
were visited on this date, were reviewed independently.
Permit Site 18
Station 268 +50 -L -, Dry Detention/ Hazardous Spill Basin. Sediment was lost off site during a heavy rain
event which caused a slope failure when water piped in beside drop inlet and channeled beside outfall
pipe. Volume of lost material was estimated to be approximately 10 cu yds and was reported to DWQ on
May 4, 2011 Clean up was in progress at the time of our visit. It was noted that approximately 250 ft of
stream and floodplains were impacted by the sediment. It was determined that concentrated sediment
deposits would continue to be removed by hand. Areas where there was only a film of sediment less than
a % inch would not be removed to limit adverse impacts to the system.
Permit Site 13
Station 147 +00 -L- LT, 36" pipe inlet, it was noted that the invert of the pipe was approximately 8 ft below
the intermittent streambed. Due to washing issues, the contractor had placed granite stone along the face
of the slope cut at the discharge point of the stream channel. It was disclosed that the stone was not in
the original permit application; however the stream channel and wetlands upstream of the culvert had
been permitted as a total take. Furthermore, filter fabric was not placed under the stone. Questions were
raised concerning the invert of the culvert at this site and the placement of the granite along the
backsiope. Consultation with the USACE and other DWQ reps who have worked on this project is
needed to determine if this work is consistent with the authorizations issued for this project.
Station 147 +60 -L- RT, 36" pipe outlet, the stones that had been discharged into the buffer area and
adjacent tributary in March during blasting had been removed. Clean up was determined to be
satisfactory and no further clean up is required.
Permit Site 14
Station 164 +00 -L -, RCBC, excessive turbidity in the stream was noted at this site. Source of turbidity
appeared to be discharge from the stilling basin on the downstream (left) side of the project. Additional
BMP's are needed in this area to reduce turbid discharge.

Permit Site 17
Station 234 +64.80 -L -, RCBC, turbid water from stilling basin at Sta. 240 +00 -L- LT was noted in
floodplain, however discharge had not reached the stream. This area needs to be monitored to ensure
discharge does not impact stream.
Station 238 +46 -L -, RCBC, the site appeared to be stable and no issues were noted.
Permit Site 25
Station 408 +50 -L -, 48" pipe, the site appeared to be stable and no issues were noted.
Station 412 +00 -L -, 48" the site appeared to be stable and no issues were noted.
Permit Site 24
Station 385 +40 -L -, 48" pipe, sediment loss had been reported on 3124111. Additional clean up is
required. Sediment accumulations (bars) were noted from the outlet of the culvert down to first log debris
jam, approximately 50 ft. These accumulations need to be removed by hand as soon as possible.
Permit Site 26
Station 427 +50 -L -, 36 "pipe, the site appeared to be stable and no issues were noted.
Permit Site 27
Station 450 +23 -L -, RCBC, the inlet looked good; a small amount of sedimentation was noted at the
outlet. An area of bank erosion was noted within the permitted area just downstream of culvert on the
right side. DWQ request that DOT inspect this site and determine if this is a result of the project and if it
is, remove the sediment from the stream and stabilize the bank. DOT may also want to investigate a
blockage /dam noted inside of the culvert.
