HomeMy WebLinkAbout20060331 Ver 1_Staff Report_20120111R -26066 (NC 311 Project) Randolph County Sites 23/24 8/13/10 First site visit, Amy Euliss and Barry Harrington present, Site 24 NOTES: sediment loss from ditch failure, clean up complete, site has had two sediment losses prior to this event. Monitor development of sediment bars and remove accordingly. 8/25/10 NOTES: Site 24 visited, small film of sediment noted in pool below limits of prior loss, removal not feasible at this time. 9/21/10 Project inspected, Sites 23/ 24 not visited 9/28/10 Received complaint from Mr. Farlow 10/6/10 Project inspected, Sites 23/24 not visited 10/25/10 Meet with Mr. Farlow and NCDOT, noted turbid water in pond, vegetation was visible on bar identified by Mr. Farlow, Geotech took bore samples, reviewed north end of project, Site 23/ 24 not visited. 3/23/11 Project inspected, Site 23/24 not reviewed. 3/24/11 Sediment loss reported at Site 24, pictures and recommended actions provided by DOT, reached impacted —150 ft. Recommended Actions to take: • Remove Silt from stream by hand to the location that we have before Observe and remove if needed further down stream deposits • Remove Silt from Pipe lines and drainage structures. • Ensure that proper Type A inlet protection are installed. • Add Rock Check with 57stone in rock lined ditch. 5/10/11 Sediment loss reported at Site 24, upon inspection noted sediment bars from pipe outlet extending 50 ft downstream and stopped at log jam. DOT instructed to remove depositions. 6/28/11 Contacted by Mr. Farlow about turbidity in his pond, conducted a site visit at pipe crossing on Mr. Farlow's property, noted some accumulation of sediment at inlet, however material appeared to be native material and not from project. Mr. Farlow and I visited a reach of stream between Farlow property and road project. Noted sediment deposition in stream, source unknown, did not see evidence of washing or erosion from logging operation that had occurred recently in area. Visited Sites 23/24 area on project, did not note any BMP failures. I noted sediment in outlet of pipe at Site 23, source unknown. 7/12/11 Investigated report of sediment loss, water had piped beside drop inlet and blew out slope and sediment deposited in stream for a reach of 150 ft to 4- wheeler crossing. DOT instructed to remove sediment. 7/19/11 Reviewed clean up of sediment at Site 24, removal completed, noted "nice job 7/25/11 Project inspected, Sites 23/24 not visited. 9/23/11 Sediment loss reported at Sites 23/24 by DOT: L- Sta: 381 +00 RT at 24" CSR - Approximately 10OLF of stream impacted with around .25CY of silt loss. (silt appears to have come from failure around the 24" CSP) L- Sta: 385 +00 RT at the end of SR3 - Approximately 150LF of stream impacted with around .5 CY of Silt loss. (Not sure where the silt is coming from) 1/11/12 Site 24 visited, no issues noted and SDO's functioning properly.
